POSITION OVERVIEW
• Home health physical therapy role focused on one-on-one patient care
• Complete evaluations, start of care visits, reassessments, and discharges
• Build meaningful long-term patient relationships
• Practice independently with leadership support available when needed
• Family-owned organization focused on clinician success and work-life balance

COMPENSATION
• $55-$58 Per Unit (Full-Time)
• $60-$65 Per Unit (PRN)
• 24 Units Guaranteed for Full-Time Status
• Strong productivity-based earning potential
• Referral Bonuses Up To $2,500
• Bi-Weekly Pay

SCHEDULE
• Flexible Self-Managed Scheduling
• No Rigid Clock-In / Clock-Out Requirements
• Create Your Own Daily Schedule
• Zoned Territories To Reduce Excessive Driving
• Full-Time Clinicians Average 30-35 Units Weekly

ROLE
• Perform physical therapy evaluations and assessments
• Manage start of care visits and treatment planning
• Complete reassessments and discharge visits
• Deliver one-on-one therapy in patient homes
• Coordinate care and maintain clinical documentation
• Develop meaningful relationships with patients and families

Requirements

• Active Florida Physical Therapist License REQUIRED
• Home Health Experience Preferred
• Comfortable Working Independently REQUIRED
• Strong Assessment and Patient Care Skills REQUIRED
• Ability to Manage Caseload Independently REQUIRED
• Excellent Communication Skills REQUIRED
