What You'll Do
As a PRN Home Health LVN, you'll work under the direction of a Registered Nurse to deliver skilled nursing care that promotes healing, independence, and improved quality of life.
Responsibilities include:
Provide skilled nursing visits according to the physician's orders and individualized plan of care.
Monitor patient conditions and promptly report significant changes to the RN Case Manager and physician.
Assist patients and caregivers with education on medications, treatments, disease management, and self-care.
Administer treatments and nursing interventions within your scope of practice.
Collaborate closely with the RN Case Manager and interdisciplinary care team.
Document all patient care accurately and in a timely manner.
Identify safety concerns and help implement interventions that reduce hospitalizations and emergency room visits.
Deliver compassionate, patient-centered care while maintaining compliance with all clinical and regulatory standards.

What You'll Bring
Required Qualifications
Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program
Current Texas Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license
Minimum of one (1) year of nursing experience in a clinical setting
Excellent communication and organizational skills
Valid driver's license, reliable transportation, and current automobile insurance
Ability and willingness to travel throughout the assigned service area
Physical Requirements: Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support 50–100 pounds, including assisting with patient positioning, transfers, and moving equipment
