Withdrawal Unit – Care Coordinator
Bridge House Withdrawal Unit, Highgate
Part-time 0.8 FTE | 12‑month maximum term
Why this role matters
The Care Coordinator plays a key role in supporting participants throughout their withdrawal journey, ensuring safe, timely and person‑centred care. This role strengthens continuity across admissions, clinical flow, discharge planning and external liaison, enabling nursing staff to focus on direct therapeutic engagement. As part of the multidisciplinary team, you will help create a consistent, coordinated and supportive experience for participants accessing the Withdrawal Management Unit.
About the role
The Care Coordinator provides coordinated clinical and operational support across the participant journey within the Withdrawal Unit. This includes managing daily clinical flow, coordinating admissions and discharges, liaising with GPs and external providers, and supporting continuity of care from pre‑admission through to post‑discharge. The role also contributes to operational processes, stock management, compliance checks and quality improvement activities.
This part‑time 0.8 FTE position sits within the Bridge House Withdrawal Unit and reports to the Nurse Team Leader.
The is a newly created role and is offered as an initial 12‑month maximum term with the potential for a longer-term role subject to business needs.
How you will make an impact
Coordinate daily clinical flow including admissions, discharges and transfers
Liaise with GPs and external providers to obtain medical documentation, prescriptions and follow‑up care
Support continuity of care across the participant journey
Maintain accurate and comprehensive clinical records in line with TSA requirements
Attend and contribute to weekly waitlist review meetings
Monitor outstanding clinical requirements and escalate barriers to admission
Support implementation of operational processes including stock management, emergency equipment checks and compliance tasks
Assist in establishing consistent systems and workflows to support sustainable service delivery
Provide day‑to‑day coordination support to nursing staff and contribute to a positive team culture
Participate in on‑call roster where applicable
What you will bring
Current AHPRA registration as a Registered Nurse (Division 1)
Experience in AOD, mental health or withdrawal settings
Experience in admissions, intake or care coordination
Ability to coordinate care in complex clinical environments
Strong communication, organisational and clinical liaison skills
Understanding of trauma‑informed and culturally responsive practice
Ability to work collaboratively across multidisciplinary and interagency teams
