The Traveling Social Services Specialist is responsible for assessing and supporting the emotional, financial, spiritual, educational, and social needs of residents/patients and their families, and for enhancing quality of life through individual, group, and interdisciplinary interventions. This role requires extensive travel to assigned centers to provide consistent, on-site support and ensure continuity of services across locations.

Qualifications
- Bachelor’s degree in Social Work required; must hold and maintain an active Social Work license in good standing, in accordance with all applicable state licensing requirements..
- Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals.
- Demonstrated ability to build and maintain professional relationships and collaborate with external agencies to address health and welfare needs.
- Ability to work effectively with diverse populations in a variety of settings, including community and long-term care environments.
- Strong critical thinking skills with the ability to evaluate case information and make informed decisions regarding appropriate interventions and procedures.
- Prior experience in long-term care required
- Must be able to travel up to 100% to assigned centers, including travel by car or air.
