We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our Pembroke clinical care team. 

This role plays a critical part in delivering high-quality patient care while supporting providers in a fast-paced urgent care environment.


Position Details:

  • Status: Full-time
  • Schedule: Monday – Friday, 8:00 a.m. – 5:00 p.m.
  • Rotating Saturdays: 9:00 a.m. – 12:00 p.m. at CHoC Urgent Care Lumberton

Description: Nurses work directly with CHoC providers and other members of the clinical and support staff to assure that the highest quality of care is delivered to CHoC patients. The nurse acts as a liaison between the patient, family and providers. The nurse implements medical orders as directed by the medical providers, makes initial assessments of patients and augments the overall patient care delivered in the office.


Duties and Responsibilities:

A. Patient Care

  • Obtaining the patient chart, review for need of visit, and politely call the patient to the clinical area.
  • Introduce yourself to the patient and explain what you are going to do.
  • When needed, perform initial evaluation of patient to include subjective history and vital signs (to include, but not limited to: weight, height, temperature, blood pressure, head circumference) as age appropriate and as requested by providers on the clinical team.
  • If providing assistance to the Nursing Assistant, make sure that any vitals taken are entered in the patient’s chart.
  • Establish and document any known allergies (type and date).
  • Locate and provide any pertinent information that may impact patient care prior to the provider’s initial evaluation (lab reports, emergency department records, etc.).
  • Assess and document the patient’s immunization status and prepare and give all necessary shots as requested by the medical provider, explaining to the patient and parent what you are going to do.
  • Provide initial assessment of the patient and apprise the medical provider of any seriously ill child.
  • Administer medications at the direction of the medical providers.
  • Enter all meds in the chart (electronic or paper).
  • Perform diagnostic testing on patients as directed by the medical providers on the clinical team (example: hearing test).
  • Complete any documentation in the medical record.
  • Provide telephone triage and give patient advice with provider input when warranted.
  • Make referrals to other providers/agencies when requested.

B. General Duties

  • Ensure that rooms are adequately stocked and cleaned on a daily basis.
  • Table paper should always be clean and pulled back before taking a child into an exam room.
  • Ensure adequate medical supplies, medications and vaccines are available and notify the Director of Materials and Facilities Management when repairs are needed.
  • All phone calls should be returned within 30 minutes, unless there are extenuating circumstances.
  • All calls must be properly documented.
  • Consistently provide immediate and appropriate attention to patients and family members.
  • Consistently demonstrate prompt and reliable attendance.
  • Consistently demonstrate cooperation and assistance with other CHC employees.
  • Consistently demonstrate positive response to scheduling changes.
  • Always protect the confidentiality of information and records.
  • Maintain professional appearance and actions.
  • Always follow mandated regulations such as HIPAA and OSHA.
  • All other duties as assigned/requested.


Requirements

 Qualifications:

  • Graduate of an accredited nursing program.
  • Licensed or eligible by the NCNB.
  • Adheres to the rules and regulations of the NCNB at all times.
  • 3 years Experience in clinical work. (pediatric nurse to include immunization preferred)
  • Attains age-specific CME on a regular and routine basis.
  • Excellent organizational skills.
  • Excellent communication skills (oral and written).
  • Empathy and the ability to relate to children
  • Must be organized and exemplify the attitude and manner of a team player.
  • Basic computer skills.
  • Electronic health records experience.

OSHA Category I:Physical demands include frequent standing, walking, bending, reaching and lifting (babies and small children, etc.).Work conditions include contact with patients in controlled environment, potentially contagious diseases and possibly bloodborne pathogens.
