What You’ll Do
As a Home Health RN, you will:
-
Complete comprehensive skilled nursing assessments
-
Perform Start of Care, Resumption of Care, Recertification, and Discharge visits as assigned
-
Complete OASIS assessments as required
-
Develop and update individualized plans of care
-
Provide wound care and other skilled nursing treatments
-
Complete medication reconciliation and medication education
-
Provide disease-management and patient education
-
Monitor patients for changes in condition
-
Communicate with physicians regarding patient needs and orders
-
Help identify concerns early to reduce avoidable hospitalizations
-
Educate patients, families, and caregivers
-
Collaborate with PT, OT, SLP, MSW, LPN, and other members of the interdisciplinary team
-
Complete accurate and timely clinical documentation using WellSky
What We’re Looking For
-
Active Maryland Registered Nurse License Required
-
Graduate of an accredited nursing program
-
Reliable transportation
-
Strong clinical assessment and communication skills
-
Ability to independently manage home health visits
-
Strong organizational and time-management skills
-
Home Health and OASIS Experience Preferred, but Not Required
-
WellSky Experience is a Plus
