What We Offer

• $17/hr.

Your Day-to-Day
You'll help residents and their families through some of life's biggest transitions. You'll support admissions and discharge planning, advocate for residents' needs and rights, help coordinate services and resources, and be a caring, steady point of contact for families. You'll work under the supervision of our Social Services Director to ensure compliance with current state, federal, and local standards. You'll do it alongside a close-knit team that truly loves working together.

What You'll Need

• SNF experience
• Strong communication and organizational skills
• A calm, caring presence with residents and families
• Compassion for a vulnerable population
• Experience in social services or a related field preferred
• PointClickCare (PCC) experience preferred
