As an Clinical Team Manager, you’ll contribute to our success in the following ways:
- Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are consistently met.
- Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
- Ensure timely start of care and ongoing compliance with applicable regulatory requirements.
- Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
- Conduct regular OASIS reviews to support accurate documentation, compliance, and quality patient care.
- Provide guidance and coaching to direct care team members and assist with interviewing and hiring clinical staff.
- Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.
What is Required?
- Associate degree in a Nursing related field required
- 2 years home care experience as a registered nurse in a Home Health or Hospice environment
- 1 year of supervisory experience preferred
- Current, unrestricted RN license valid for the state of work
- Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
