About the Role
As a Primary Care Physician, you will serve as the cornerstone of care for an established panel of adult and geriatric patients. Working alongside a dedicated interdisciplinary care team, you will focus on improving health outcomes, preventing hospitalizations, and helping patients successfully manage chronic conditions.
Common conditions managed include:
- Diabetes
- Hypertension
- Congestive heart failure
- Coronary artery disease
- COPD and respiratory disorders
- Chronic kidney disease
- Arthritis and mobility limitations
- Multiple chronic comorbidities common within the Medicare population
This role is ideal for a physician who values continuity of care, patient education, and long-term relationships.
Responsibilities
- Provide comprehensive primary care services for adult and geriatric patients
- Diagnose, treat, and manage acute and chronic medical conditions
- Develop individualized care plans focused on prevention and wellness
- Conduct annual wellness visits and preventive screenings
- Coordinate care with specialists, hospitals, and ancillary providers
- Educate patients and families on disease management and healthy lifestyle choices
- Participate in quality and outcome-driven initiatives
- Collaborate with a multidisciplinary care team to support patient success
Qualifications
Required
- MD or DO degree from an accredited medical school
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active Florida medical license or ability to obtain
- DEA certification
- Commitment to high-quality, patient-centered care
Preferred
- Experience caring for Medicare and senior populations
- Chronic disease management experience
- Value-based care experience
- Bilingual English/Spanish skills a plus

$192,153.00to
$288,230.00
The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
