Description
- Conduct scheduled face-to-face follow-up assessments with assigned clients.
- Obtain and document vital signs and relevant client information.
- Observe and assess the client’s health, activity level, functioning, safety, and overall well-being.
- Review the client’s current care plan and identify changes in needs or services.
- Document assessment findings accurately and timely.
- Identify and report significant changes or concerns to agency leadership.
- Recommend care-plan updates when appropriate and within the LPN’s scope of practice.
- Communicate professionally with clients, caregivers, and agency staff.
- Maintain client confidentiality and comply with agency policies and applicable regulations.
- Promote client dignity, independence, safety, and quality of life.
- Current, unrestricted Georgia LPN license.
- Home health, community care, or related experience preferred.
- Strong assessment, documentation, and communication skills.
- Ability to work independently and meet documentation deadlines.
- Reliable transportation to assigned client homes.
- Ability to maintain professional standards and confidentiality.
Primary Purpose: To ensure clients receive appropriate follow-up monitoring and that their care plans remain current, accurate, and responsive to their needs.
