Overview
Health at Home Nurse Practitioner (FT) APRN
The Nurse Practitioner (APRN) is a registered professional nurse with advanced graduate education and clinical training who supports comprehensive home-based care services with a focus on end-of-life care, including palliative care and hospice, in compliance with the state nurse practice act. The APRN conducts holistic assessments of assigned patients to contribute to the overall evaluation of the patient’s care needs and the patient’s goals of care. Participates as part of the hospice medical team by completing face-to-face visits, acting as an attending provider and is a contributing member of the hospice interdisciplinary team. Serves as an active member of the Section of Palliative Medicine at Dartmouth-Hitchcock, with an academic appointment in the Department of Medicine, participating in the education of Hospice and Palliative Medicine fellows and rotating learners, and collaborating with Palliative Medicine team members in the care of mutual patients.
Qualifications
- Board-certification by appropriate accrediting body to practice as an APRN.
- Must have an active national certification as an Adult Nurse Practitioner, Adult- Gerontology Primary Care Nurse Practitioner or Family Nurse Practitioner.
- The ideal candidate will have completed advanced practice fellowship training in palliative care, OR will have at least 2+ years of experience practicing in hospice palliative care.
- Certification in Advanced Practice Palliative Nursing or be eligible for certification within one year.
