Description
Physical Therapist
FLSA: Exempt
Reports To: Clinical AD
Physical Requirements: The ability to transfer people with disabilities using approved techniques bending, walking, standing, lifting 50lbs, kneeling is also required.
Hours of Work: Monday thru Friday, 8:30am to 4:30pm. with flexible schedule to allow for time spent in residences early evening and early morning. Fulltime 37.5 hours per week (Weekend, on-call or holiday work as needed)
Job Summary: Provide physical therapy assessments and treatment to ensure clinical and regulatory compliance. Schedule and provide therapy in the Article 16 clinic, as well as in Day and residential programs as needed.
Job Qualifications:
- Individual must be licensed and currently registered as a Physical Therapist by the NY State Department of Education.
- Specialized training or one year of professional experience in treating developmentally disabled preferred
- Valid NY State driver's license required. Some travel required.
Major Responsibilities:
- Provide Physical Therapy services to individuals in programs, in compliance with NYS, Federal, agency and department regulations.
- Provide training to agency personnel to include in home and work sites.
- Record and maintain individual's PT record in compliance with all state, federal, local regulations and agency policy.
- Participate in team meetings and surveys as part of individuals overall plan of care.
- Communication that is appropriate, accurate and timely.
- Pursue personal/professional development and attend mandated agency training as required.
- Monitor all adaptive equipment and schedule repairs or replacement as needed for wheelchairs, walkers, and orthotics.
