Community Regional Medical Center (CRMC) is the flagship hospital of Community Medical Centers – a not-for-profit healthcare system based in Fresno, California, and the largest healthcare provider and private employer in central California. The 56,000 square-foot emergency department is one of the largest and busiest in the state. We have an 84 bed Level 3 NICU and a brand new pediatric specialty care unit. Future plans include building a women and children’s facility on campus and expanding with 65 additional beds.

Responsibilities

The Pharmacist provides advanced medication therapy management (MTM) services within referral-based Hospital Outpatient Department (HOPD) pharmacist-managed clinics. Practicing under a collaborative practice agreement (CPA) and in accordance with the California State Board of Pharmacy, the pharmacist manages complex and chronic conditions by initiating, adjusting, and discontinuing medications; conducting patient assessments; and monitoring therapeutic outcomes. The role includes collaborating with prescribers under a CPA, coordinating medication access, promoting adherence through patient education, documenting care in the electronic health record (EHR), and supporting quality improvement and professional development initiatives. Job DescriptionEssential AccountabilitiesReflects the job's main responsibilities and is not intended to be an exhaustive list of all duties performed; therefore, its content does not restrict management's right to assign or reassign duties and responsibilities to individuals in this job.o Provides comprehensive medication management services within HOPD referral-based pharmacist-managed clinics.o Initiates, modifies, and discontinues medication therapy under a CPA and in accordance with state regulations.o Conducts comprehensive patient assessments, including medication reconciliation and review of medical history under a CPA.o Orders, interprets, and monitors laboratory tests to ensure medication efficacy and safety under a CPA.o Manages complex and high-risk chronic disease states (such as oncology supportive care, weight loss, diabetes, hypertension, heart failure, anticoagulation, COPD and hyperlipidemia) under a CPA.o Develops and implements individualized, evidence-based treatment plans aligned with clinical guidelines under a CPA.o Monitors therapeutic outcomes and adjust pharmacotherapy to achieve optimal clinical goals under a CPA while identifying and resolving medication-related problems.o Provides patient education and counseling to improve medication adherence and support disease self-management through in-person or virtual visits.o Collaborates with referring providers, interdisciplinary teams, payers, retail pharmacies, and specialty pharmacies to coordinate care and address medication access issues.o Documents all patient encounters and clinical interventions in the electronic health record (EHR) and track clinical outcomes and quality metrics to support value-based care and quality improvement initiatives.o Serves as a preceptor for pharmacy students and residents when applicable.o Performs other job-related duties as assigned. Knowledge, Skills, and Abilitieso Strong pharmacotherapy knowledge for chronic and complex conditions.o Understanding of state pharmacy regulations, scope of practice, and collaborative practice agreements.o Familiarity with evidence-based guidelines, lab monitoring, and medication therapy management.o Ability to assess patients, review medication regimens, and participate in development of individualized treatment plans under a CPA.o Supports medication initiation, modification, and discontinuation per collaborative agreement protocols.o Patient education and counseling, including virtual care delivery.o Proficiency with Epic or similar EHR systems (e.g., Epic) for documentation and tracking clinical outcomes.o Strong communication, problem-solving, and interdisciplinary collaboration skills.o Ability to adapt care delivery to diverse patient populations and telehealth environments.o Commitment to patient-centered care and quality improvement initiatives.

Qualifications

Educationo Doctor of Pharmacy (PharmD) Degree from an ACPE-accredited institution required Experienceo PGY-1 Pharmacy Practice Residency or significant ambulatory clinical experience requiredo PGY-2 residency in ambulatory care or related specialty preferredo Advanced clinical expertise in chronic disease management preferredo Experience or familiarity with 340B program operations and compliance preferredo Demonstrated pharmacy practice experience in ambulatory or HOPD pharmacist-managed clinics preferred Licenses and Certificationso Current and unrestricted Registered Pharmacist (RPh) License in California requiredo Board certification (BCPS, BCACP, or equivalent) required within 18 months of hireo Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
