$110,000-$125,000 Annual Salary

Position Summary

The Nurse Practitioner (NP) or Physician Assistant (PA) reports to and under the guidance of the Site Medical Director and collaborating physician, in accordance with the Heartland Health Services’ (HHS) mission, strategic goals, federal and state laws and regulations, performance and outcome objectives, and accreditation standards.   

The NP or PA provides comprehensive primary care services to patients across a variety of settings, including HHS's Mobile Health Unit serving rural and underserved communities. This position plays a vital role in expanding access to healthcare by delivering preventive, acute, and chronic disease management services directly to patients where they live and work. The provider must be comfortable practicing independently within scope, working in a mobile environment, traveling to community locations, and collaborating with interdisciplinary care teams to improve health outcomes and reduce barriers to care. 

Essential Functions

• Provides primary care services on the Mobile Health Unit four days per week and at a clinic site one day per week. 
• Travels to designated rural and community-based service locations to deliver healthcare services and support HHS outreach initiatives. 
• Assesses the health status of patients by obtaining relevant medical histories and performing comprehensive physical examinations appropriate to age and presenting condition. 
• Establishes priorities and develops patient-centered plans of care, including ordering diagnostic tests, prescribing medications, recommending non-pharmacologic interventions, and providing patient education. 
• Diagnoses and manages acute and chronic illnesses in accordance with HHS protocols and evidence-based practice standards. 
• Conducts and interprets diagnostic tests, prescribes treatments, provides health education, and makes referrals to specialists, community resources, and other healthcare providers as appropriate. 
• Maintains systematic follow-up processes to evaluate treatment effectiveness, modify care plans as needed, and ensure continuity of care. 
• Accurately documents patient encounters, treatment plans, and outcomes in the electronic health record while maintaining confidentiality and compliance with applicable regulations. 
• Participates in periodic quality assurance and quality improvement activities, including chart reviews and outcome monitoring. 
• Collaborates with physicians, nurses, care coordinators, behavioral health providers, and other team members to provide comprehensive, patient-centered care. 
• Supports HHS community outreach efforts and initiatives designed to improve healthcare access in rural and underserved populations. 
• Functions as a role model and preceptor for nurse practitioner, physician assistant, or other healthcare professional students when applicable. 
• Participates in HHS collaborative projects, peer review activities, and organizational initiatives. 
• Maintains current licensure, certification, and continuing education requirements in compliance with state and federal regulations. 
• Develops and maintains positive working relationships with patients, families, community partners, staff, and outside agencies. 
• Ensures compliance with all federal, state, local, and HHS policies, procedures, and regulations related to clinical care, workplace safety, public health, and patient confidentiality. 
• Supports HHS continuous quality improvement efforts aimed at enhancing patient outcomes, patient satisfaction, and operational efficiency. 
• Assists in maintaining Mobile Health Unit readiness, including reporting equipment, technology, supply, or vehicle concerns through appropriate channels. 
• Attends staff meetings, department meetings, and other required organizational meetings. 
• Performs other duties as assigned to support the mission and operational needs of Heartland Health Services.

Requirements

• 1-2 years’ work experience as a Nurse Practitioner or Physician Assistant providing primary care in community health, rural health, public health, family practice, or Federally Qualified Health Center (FQHC) settings required.
• Requires an advanced professional level of knowledge of healthcare practice, clinical assessment, diagnosis, and treatment which can be acquired through completion of an accredited Nurse Practitioner or Physician Assistant program and maintenance of appropriate state licensure and certification. 
• Current State license as a Registered Nurse and licensed Nurse Practitioner or Physician Assistant and current DEA license. 
• Ability to demonstrate the knowledge and skills necessary to provide patient care appropriate to the age of the patients served.  
• Ability to demonstrate the principles of growth and development as well as the physical, emotional, and psychosocial needs of the patient population served. 
• Strong interpersonal skills necessary to educate patients and their families and provide emotional support during a crisis; deal effectively with physicians; and provide training and education to residents, medical students, and nurses. 
• Strong analytical ability to assess condition of patient based on physical examination, and appearance and laboratory and related clinical data; develop patient care plans and monitor progress toward goals; and recognize and initiate emergency interventions as appropriate. 
• Basic Microsoft Word and Excel knowledge preferred but not required. 
• Electronic Medical Records (EMR) experience preferred but not required. 
• Ability to work independently and adapt to varying clinical environments.
