About the Opportunity
This role responsible for ensuring Transformation is planned, communicated, coordinated and adopted across HammondCare's initiatives.
Working within the Transformation Office, the role supports the people side of the business by helping leaders and teams prepare for, implement and embed new ways of working. The role assesses those impacted by Transformation, identifies affected stakeholders, develops practical change and communication plans, supports business readiness, and provides leaders with the tools and guidance required to successfully navigate transformation.
The role works closely with initiative owners, business leaders and Marketing to ensure stakeholders understand what transformation is, why it is happening, and what support is needed to enable adoption. It monitors readiness and adoption throughout implementation, identifies barriers to successful uptake, and works with leaders to address issues that may affect outcomes.
At an enterprise level, the role maintains visibility of transformation activity across the portfolio, helping identify cumulative impacts, competing demands and adoption risks across business units and services. The role ensures significant transformation impacts and stakeholder risks are visible and managed, while capturing lessons learned to continuously improve HammondCare's approach to change and adoption.
This is a maximum term contract role for 22 months based at St Leonards with hybrid working opportunities.
How you’ll make an impact:

Change Impact & Readiness: Assessing how transformation initiatives will affect people, teams and ways of working, and ensuring appropriate support is in place before change is implemented.

Implementation & Adoption Support: Coordinating activities that help teams transition to new processes, systems and ways of working and supporting adoption after go-live.
Adoption Monitoring & Reinforcement: Measuring adoption outcomes, identifying barriers to sustained transformation and working with leaders to address issues impacting uptake.
Enterprise Transformation Visibility: Maintaining an organisation-wide view of transformation activity to identify cumulative impacts, competing demands and stakeholder risks across HammondCare.
Continuous Improvement: Capturing lessons learned and using insights from implementation and adoption activities to strengthen future transformation approaches and support better outcomes.
