Job Description:

The HealthySteps Specialist is an early childhood development professional embedded within the pediatric primary care team. This role focuses on supporting children ages 0 to 3 and their families through prevention-focused, relationship-based care that promotes healthy development and strengthens caregiver-child relationships.

HealthySteps is an evidence-based model of care that integrates behavioral health and child development expertise directly into pediatric primary care. The program supports social-emotional development, language and cognitive growth, and overall family well-being during the most critical years of a child’s development.

Working closely with pediatric providers and the care team, the HealthySteps Specialist provides early identification of developmental and behavioral concerns, delivers brief interventions, and connects families to appropriate resources. Based on clinic needs and caseload, this role may also support older pediatric patients ages 3 and older to ensure continuity of care across the clinic population.

Essential Functions 

  • Participates in team-based well-child visits to support development, behavior, and relational health 
  • Provides anticipatory guidance on development, sleep, feeding, behavior, and positive parenting 
  • Conducts developmental and social-emotional screenings and assessments 
  • Delivers brief consultation and short-term interventions for behavioral and developmental concerns 
  • Supports families in strengthening caregiver-child relationships and promoting healthy development 
  • Follows up with families to reinforce care plans and ensure ongoing engagement 
  • Identifies needs and connects families to appropriate community resources and services 
  • Provides warm handoffs and tracks referrals to ensure successful connections 
  • Collaborates with pediatric providers, advanced practice providers, and clinic caregivers to coordinate care 
  • Participates in team meetings, case reviews, and quality improvement activities 
  • Provides education and consultation to clinic caregivers related to early childhood development 
  • Documents all patient interactions and care coordination in the electronic health record 
  • Tracks program metrics including patient engagement, referrals, and outcomes 
  • Supports program implementation and ongoing development within the clinic 

Minimum Qualifications

  • Current licensure in the state of practice as a Licensed Clinical Social Worker (LCSW), Clinical Mental Health Counselor (CMHC), or Marriage and Family Therapist (MFT). Current licensure in LLMSW, CSW, ACMHC, or AMFT in state of practice acceptable but must obtain LCSW, CMHC, or MFT within one (1) year of hire or promotion into this role.
  • Master’s degree in Social Work, Psychology, Counseling, or a related field from an accredited institution (degree verification required).
  • Demonstrated experience in behavioral health care, preferably in a collaborative care setting.
  • Strong understanding of evidence-based therapeutic techniques and interventions.
  • Excellent verbal and written communication skills.
  • Ability to work effectively within a multidisciplinary team.
  • Proficiency in electronic health records (EHR), documentation, and Microsoft 365 products.
  • Commitment to patient advocacy and providing patient-centered care.

Preferred Qualifications

  • Current licensure in the state of practice as a Licensed Clinical Social Worker (LCSW), Clinical Mental Health Counselor (CMHC), or Marriage and Family Therapist (MFT).
  • Experience and expertise in clinical practice within an ambulatory care setting, community health, or home care.
  • Experience working successfully in a remote environment, including proficiency with advanced Microsoft Suite tools such as Teams (chat, whiteboard, task tracking) and Microsoft Outlook.
  • Experience in care management, care navigation, or a closely related field such as discharge planning, managed care, behavioral health, or patient education.
  • Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment.
  • Strong written and verbal communication skills.

Physical Requirements

  • Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess customer needs.
  • Frequent interactions with customers that require employee to communicate as well as understand spoken information, alarms, needs, and issues quickly and accurately.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer, phone, and cable set-up and use.
  • Expected to lift and utilize full range of movement to transport, pull, and push equipment. Will also work on hands and knees and bend to set-up, troubleshoot, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
  • For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$43.09 - $66.47
