Avance Care is committed to delivering exceptional care with a strong emphasis on patient involvement in preventive care and treatment plans. Requirements: Nurse Practitioner or Physician Assistant with an active North Carolina State License Location: Cary-Preston, NC Explore and experience how Avance Care is providing professional and personal life balance:

• Maximum of 40 hours per week (including breaks and administrative time)
• Expected average patient load of 2 visits per hour
• Collaborative environment with onsite lab, behavioral health therapists, and nutritionists
• Innovative, integrated use of telehealth
• Centralized care coordination, care management, and population health management
