Licensed Practical Nurse (LPN) / Registered Nurse (RN) | FT/PT/PRN | Private Duty Nursing | Madison, TN
All Heart Home Care is seeking compassionate and dependable Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our Private Duty Nursing team in the Madison, TN area. We are currently hiring for Full-Time (FT), Part-Time (PT), and PRN positions with opportunities available on days, nights, weekdays, and weekends.
This position offers the opportunity to provide one-on-one skilled nursing care in the comfort of the patient's home. We are seeking dedicated clinicians who are committed to delivering high-quality, patient-centered care while building meaningful relationships with patients and their families.
Pay: $30.00 - $35.00 per hour
Key Responsibilities
- Provide skilled nursing care in the patient's home according to physician orders and the established plan of care
- Monitor patient conditions and report changes to the appropriate members of the healthcare team
- Administer medications, treatments, and nursing interventions as ordered
- Assess and document patient status, progress, and response to treatment
- Educate patients and caregivers regarding medications, treatments, and ongoing care needs
- Collaborate with physicians, caregivers, and interdisciplinary team members to ensure continuity of care
- Maintain accurate and timely clinical documentation
- Promote patient safety, comfort, dignity, and independence within the home environment
Qualifications
LPN Applicants
- Current Licensed Practical Nurse (LPN) license in the State of Tennessee
- Current CPR/BLS certification
RN Applicants
- Current Registered Nurse (RN) license in the State of Tennessee
- Current CPR/BLS certification
All Applicants
- Strong communication, organizational, and clinical skills
- Ability to work independently in a home care setting
- Dependable, compassionate, and patient-focused
- Previous home health or private duty experience preferred but not required
- Must pass a background check and drug screening
