In addition to supporting individual patients, the Nurse Navigator contributes to clinical quality, cost-effective care, and program performance by monitoring outcomes, identifying improvement opportunities, developing educational initiatives, and collaborating with providers and multidisciplinary teams. This role requires strong clinical judgment, independence, and a commitment to delivering high-quality, patient-centered care.

Your Day-to-Day Responsibilities

Patient Navigation & Advocacy

  • Screen patients to identify appropriate candidates for navigation services and manage an assigned patient caseload.
  • Guide patients through the healthcare system by coordinating referrals, facilitating access to specialty care, and connecting patients with clinical, financial, and community resources.
  • Assess patient and family needs, including physical, psychosocial, emotional, and financial barriers that may affect the plan of care.
  • Coordinate referrals to appropriate services, including nutrition, social work, home health, hospice, complementary therapies, and community resources.
  • Develop therapeutic relationships, promote patient satisfaction, and advocate for the rights, dignity, and needs of patients and their families.
  • Track interventions and outcomes, collaborate with the healthcare team, and address barriers to continuity of care.

Clinical Oversight & Care Coordination

  • Monitor treatment plans and notify the appropriate provider when a plan of care is ineffective or requires reassessment.
  • Apply clinical knowledge and nursing judgment to support safe, appropriate, and effective patient care.
  • Work independently within the scope of RN licensure and under the direction of the physician of record.
  • Serve as a clinical resource for hospital- and clinic-based nursing teams.
  • Participate in multidisciplinary conferences and collaborate with physicians, nursing staff, management, and other care team members to coordinate care.
  • Participate in payer appeals by applying relevant review criteria and patient-specific clinical information.

Quality Improvement & Program Development

  • Collect, trend, and analyze patient outcome data to identify best practices, evaluate program effectiveness, and support performance improvement.
  • Collaborate with program leadership to establish key performance indicators and improve clinical quality, patient outcomes, and care delivery.
  • Contribute to the development and review of navigation program standards, competencies, guidelines, policies, and training plans.
  • Identify opportunities to develop or enhance programs in response to community needs.

  • Develop and deliver education plans and materials that support patient understanding, informed decision-making, and effective care management.
  • Provide staff education, in-services, and learning opportunities that support clinical practice improvement.
  • Participate in community awareness and outreach activities to promote education and access to available resources.

Professional Collaboration & Accountability

  • Establish effective working relationships with patients, families, providers, community agencies, and other healthcare professionals.
  • Demonstrate initiative, sound judgment, organization, and adaptability when managing competing priorities and resolving complex issues.
  • Support responsible resource allocation and organizational goals through sound clinical and fiscal stewardship.
  • Maintain reliable attendance, meet organizational competency expectations, follow CSC policies and procedures, and demonstrate the organization's core values of respect, integrity, stewardship, excellence, collaboration, and kindness.
