Purpose of this position
The Case Manager will manage a caseload of program participants receiving services from our Developmental Disabilities Administration (DDA) and Division of Rehabilitation Services (DORS). This position is responsible for creating and implementing realistic employment goals based on person-centered planning (PCP) and promoting meaningful day and community membership through employment, volunteer opportunities, and social/recreation activities. The purpose of the position is to promote and explore community employment through an individual discovery process. This position will work in conjunction with the Day and Employment Program Directors and Program instructors to build skills for competitive integrated employment in the community.
Duties and Responsibilities
Case Management
- Responsible for training Direct support professionals (DSP’s) on participants' Person-Centered Plan(PCP).
- Monitoring participants' PCP goal progress with the focus on Employment First and Community membership.
- Will work as an Employment specialist to interact with the business community to secure employment opportunities for participants.
- Research job opportunities on the internet and in the local community.
- Facilitate discovery activities for the purpose of employment.
- Participate in annual PCP meetings and develop strategies to meet goals related to employment first and meaningful day services.
- Create and monitor tracking of individual PCP goals and related schedules.
- Documentation and billing per DDA and DORS regulations.
- Cover Day program and Community Development services as needed.

Employment support
- Assist individuals to identify interests and abilities through discovery and help make choices about employment opportunities.
- Create Discovery Profiles, individual schedules, Personal Data sheets and resumes for DDA-funded individuals.
- Assist individuals to submit applications, resumes, cover letters, and references in person and online as appropriate.
- Assist individuals to follow up on job opportunities.
- Job coach
- Documentation and billing per DDA and DORS regulations.
Requirements
Qualifications
- Proficiency with Windows applications, primarily Word and Excel
- Excellent organizational skills, accuracy, dependable work habits, and good communication skills
- Must pass a criminal background check and have a clean driving record
- Must possess a Bachelor’s degree or equivalent experience in case management
- Certified Med Tech within 6 months of hire as needed
- Certified Employment Support Professional (CESP) certified within one year of hire
