Description
The ideal candidate is dedicated to helping residents achieve their highest level of communication, cognitive function, and swallowing safety while providing exceptional, resident-centered care.
Position Summary:
Responsibilities:
- Evaluate residents with speech, language, cognitive, communication, and swallowing disorders.
- Develop individualized treatment plans based on physician orders and clinical assessments.
- Provide skilled speech therapy services in accordance with professional standards and facility policies.
- Perform bedside swallowing evaluations and make dietary recommendations in collaboration with the interdisciplinary team.
- Educate residents, families, and caregivers regarding treatment plans and home exercise programs.
- Maintain accurate, timely documentation in compliance with Medicare, Medicaid, PDPM, and facility requirements.
- Participate in interdisciplinary care plan meetings and discharge planning.
- Communicate resident progress and changes in condition to physicians, nursing staff, and rehabilitation team members.
- Ensure compliance with all federal, state, and facility regulations.
Qualifications:
- Current New York State Speech-Language Pathologist license required.
- Certificate of Clinical Competence (CCC-SLP) preferred; Clinical Fellowship candidates may be considered.
- Experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred.
- Knowledge of dysphagia management, cognitive therapy, communication disorders, and PDPM is preferred.
- Strong communication, organizational, and documentation skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
Schedule:
- Part-Time.
- 20 hours per week.
