CO State Veterans Community Living Center at Rifle
You are required to work at your assigned work location during your scheduled work times.
Monday thru Friday, 8:00 am - 4:30 pm
Please note: Work arrangements are subject to change at any time.
The position exists to assist the Medical Records Technician in performing all functions of the health record program. This position schedules all health care appointments and coordinates the transportation schedule, as well as provide notification to family/resident of appointments. Follows up with physician offices to obtain reports for records. Maintains census reporting in absence of Medical Records Technician. Tracks physician visits (Regulatory and History & Physicals) needed to be performed by physicians on a monthly basis
Client care in medical clinic/outside appointments:
• Transports residents to and from appointments outside the facility
• Communicates information with medical providers and appropriate facility staff regarding the current status of person served.
• Administers oxygen and monitors vital signs of client.
• Supervises clients in the medical clinic individuals until the appointment is completed.
• Prepares residents for examination such as transferring residents out of their wheelchairs unto an exam bed and obtaining vital signs (temperature, pulse, blood pressure and respiration rate) and weighing the resident.
• Protects the rights of persons served and enhances their quality of life
Direct Resident Care:
• Provide direct resident care including bathing, dressing, undressing, assisting in and out of bed, performing personal hygiene, feeding programs, toileting programs, range of motion, transporting, and interacting socially to meet residents’ basic needs.
• Explaining patient care before, during and after contacts; answering call lights; and assessing basic resident care needs; updating noted resident changes; recommending routines and schedules to IDT Team. Report to the charge nurse resident’s change of condition.
Basic Observation:
Perform basic resident observations, vital signs, blood pressure, weight, intake and output, basic skin assessments
Infection Control:
Provide infection control, implementing/using universal precautions, disposal of waste. Clean linen room and equipment, stock supplies, complete supply forms.
Devices and Training:
• Correct use of safety/positioning devices, side rails, transfer belts, proper use of equipment, attend on-going staff education programs.

Education/Licensure/Certification/Experience:
A valid nurse aide certification (CNA) from the Colorado Department of Regulatory Agencies for the high school diploma or GED ~or~ A valid practical nursing license (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact for the nurse aide certification (CNA) can be substituted for the education and work experience
At least one (1) year of professional experience as a CNA or performing similar duties/assignments of a CNA.
Must have valid Drivers license
Substitutions:
Current, valid certification as a Nurse Aide (CNA) from the Colorado Department of
Regulatory Agencies will substitute for the high school diploma or GED
Special Qualifications
- Required to obtain CPR certification and Vaccinations as required by State and facility.
Preferred Qualifications
- Knowledge and ability to appropriately carry out assigned resident care based on the residents' conditions; to use judgment and to accurately recognize, report and
COWINS Partnership Agreement: Relevant years of State service should be considered as a preferred qualification and given weight under the process. Part-time relevant years of State service will be prorated.
TRANSFERS ONLY: Relevant years of State service will be considered as a preferred qualification and given the weight of at least 15% of the overall score under the process.

Previous experience transporting patients, residents, or individuals in a healthcare, long-term care, or rehabilitation setting.
Experience providing excellent customer service while interacting with patients, residents, families, and staff.
Knowledge of safe patient handling and transfer techniques.
Experience operating wheelchairs, stretchers, lifts, and other patient transport equipment.
Ability to effectively communicate with individuals from diverse backgrounds, including those with cognitive or physical impairments.
Strong organizational skills and ability to prioritize multiple transport requests.
Demonstrated ability to work independently and as part of a team.
Basic computer skills for documenting and utilizing electronic medical records.
Experience working with older adults, veterans, or individuals with disabilities.
Excellent attendance record and demonstrated reliability
Experience working in a skilled nursing facility, assisted living, or veterans’ home.
Knowledge of infection prevention and control practices.
Commitment to providing person-centered, compassionate care while maintaining resident dignity and privacy.
Experience supporting emergency preparedness procedures and safe resident evacuation when needed.
- Full Background: CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)
- Must pass a TB test for employment
This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.
Motor Vehicle Check , if the employee will drive a state-owned vehicle, either in an on-going capacity while performing their normal day-to-day job duties OR on occasion for training/meetings/client visits/etc.
Must possess a valid, non-restricted Colorado Driver’s License or an non-restricted U.S. Driver's License for positions within 30 miles of the CO state border
Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.
