Occupational Therapist – Rehabilitation Services, Wheaton, Minnesota
A growing healthcare rehabilitation organization is seeking a compassionate and motivated Occupational Therapist to join its multidisciplinary therapy team. This opportunity is ideal for professionals who are passionate about helping individuals improve function, independence, and quality of life across a variety of care settings.
About the Role
- Provide occupational therapy services to patients across multiple care settings
- Evaluate patient needs and develop individualized treatment plans
- Collaborate with physicians, therapists, nursing staff, and caregivers to achieve optimal outcomes
- Deliver evidence-based interventions focused on improving functional independence
- Monitor patient progress and modify treatment plans as appropriate
- Educate patients and families regarding treatment goals and home programs
Responsibilities
- Conduct occupational therapy evaluations and assessments
- Develop and implement individualized treatment plans
- Provide direct patient care in outpatient, inpatient, and community-based settings
- Maintain accurate and timely clinical documentation
- Participate in interdisciplinary care planning and team meetings
- Educate patients, families, and caregivers on therapeutic strategies
- Ensure compliance with professional standards and regulatory requirements
- Support quality improvement initiatives and best practices
Qualifications
- Graduate of an accredited Occupational Therapy program
- Active Occupational Therapy license or eligibility for licensure in Minnesota
- Strong clinical, communication, and interpersonal skills
- Ability to work independently and collaboratively within a multidisciplinary team
- Commitment to delivering exceptional patient-centered care
- Previous rehabilitation experience preferred but not required
