- Provide inpatient physical therapy services to support mobility, functional recovery, and discharge planning.
- Occasionally provide coverage in skilled nursing facilities (SNF) and home health settings, depending on patient needs and departmental requirements.
- Support rehabilitation services for a diverse patient population, including adults, older adults, and pediatric patients when assigned and appropriately qualified.
- Monitor patient progress, reassess functional outcomes, and modify treatment plans to support continued improvement.
- Educate patients and caregivers regarding therapeutic exercises, mobility techniques, injury prevention, safety precautions, and home exercise programs.
- Collaborate with physicians, occupational therapists, nursing staff, case managers, and other healthcare professionals to coordinate comprehensive rehabilitation care.
- Maintain accurate and timely evaluations, treatment documentation, and progress notes using Epic EMR.
- Participate in interdisciplinary discussions, treatment planning, and discharge coordination when appropriate.
- Follow established clinical protocols, patient safety standards, and applicable regulatory requirements.
- Participate in weekend coverage approximately every 6–7 weeks and occasional holiday coverage as required.
