Longview is seeking an experienced Human Resource (HR) Manager to lead and manage the organization’s human resources functions. This role is critical in ensuring that Longview has the necessary staff and culture to achieve its mission, while ensuring compliance with employment laws, fostering employee development, and promoting an inclusive, diverse, and equitable work environment.

The Clinical Care Coordinator (LPN) works under the supervision of a Registered Nurse (RN) and acts as first-line supervisor to resident aides, medication technicians, and Licensed Practical Nurses on shift. This position provides assistance in the oversight of residents’ overall activities of daily living. Primary responsibilities include monitoring and assisting residents with medications, alerting relevant individuals of changes in resident conditions, and following up with all resident care issues while maintaining clinical quality assurance in accordance with New York State regulations and adherence to Longview's risk management practices. This role also assists residents with daily personal care and supports resident engagement in social and recreational activities which promote their physical and emotional health.

Responsibilities/Duties:

· Maintain consistent, high-quality care and service delivery across all shifts, including supporting residents with social, recreational, and daily living activities;

· Conduct regular rounds of Assisted Living and Enhanced Assisted Living floors to ensure resident safety and well-being, and monitor nurse call systems for timely responses;

· Observe and document residents' physical and emotional status, reporting changes promptly to the Case Manager, Registered Nurse, Physician, and/or Administrator;

· Administer medications accurately according to physician orders, ensuring proper documentation and follow-up;

· Supervise and train medication aide staff on medication management, pharmacy cycle fills, and compliance protocols;

· Conduct periodic medication aide skills assessments, medication audits, and follow up on clarifications, pending orders, and out-of-stock medications;

· Maintain accurate records in the Electronic Medical Record, including progress notes, observation forms, incident reports, physician orders, MAR/TAR, hospital/ER discharges, and other relevant documentation;

· Communicate effectively with residents, families, healthcare providers, emergency services, law enforcement, regulatory agencies, and other stakeholders as needed;

· Report accidents, injuries, and other incidents immediately and assist staff with proper documentation;

· Oversee the work of direct care staff, including resident/medication aides, as a first-line supervisor;

· Guide team members to uphold clinical excellence, quality assurance, and adherence to facility policies and scope of practice or work in New York;

· Participate in staff meetings, in-services, care plan meetings, and treatment team meetings;

· Conduct audits of medication carts, resident charts, resident rooms, and other required Assisted Living documentation on a weekly/monthly basis or as requested;

· Assist with admission paperwork, auditing medical/evaluation forms, DME orders, and other documentation as needed;

· Maintain resident listings for Department of Health, Mental Health, special DME needs, toileting, and other pertinent information;

· Maintain required certifications and licensure relevant to the job description;

· Exhibit professionalism, teamwork, honesty, and respect in all interactions with coworkers, residents, and other stakeholders;

· Fulfill emergency staffing needs as necessary;

· Perform other duties as assigned by the Administrator or Registered Nurse.

Requirements

• Must hold a valid LPN license in the state of New York and be in good standing;
• Maintain current CPR/First Aid certification;
• One (1) year of relevant experience or equivalent combination of education, training, and experience in a medical or healthcare setting;
• Demonstrate strong leadership skills including good judgment, problem-solving, and decision-making capabilities;
• MAT  (medication administration training) or willingness to obtain;
• Must be able to communicate clearly, concisely, and with empathy to build rapport and achieve high quality of care;
• Possess excellent written and verbal communication skills;
