Position Overview

The City Care Housing Stability Advocate’s primary role is to work on a collaborative team of 

Housing Navigators embedded at the Diversion Hub to provide home visits and wrap-around support 

services to individuals who have recently transitioned out of an experience of homelessness. The 

Housing Stability Advocate will help these individuals set goals and objectives within a psychosocial 

rehabilitation framework that allows for successful housing stabilization and retention.  

Key Responsibilities:

•  Carry out the mission, vision, and values of City Care.  
•  Build and maintain knowledge of community resources and develop relationships with 
• service providers that support resident needs.  
• Conduct regular home- and community-based visits to support housing stability, 
• independent living, and overall well-being.  
•  Partner with residents to identify self-directed goals, develop individualized service plans, 
• and monitor progress over time.  
• Support residents in areas that impact quality of life, including physical and mental health, 
• recovery, financial wellness, employment, education, relationships, community 
• involvement, and independent living.  
•  Provide case management, resource coordination, crisis intervention, advocacy, and direct 
• support based on each resident’s individual needs.  
•  Work directly with residents, City Care Property Management, and appropriate City Care 
• programs to identify and address concerns that may impact housing stability, facilitate 
• effective communication, and develop collaborative solutions that support successful 
• tenancy.  
• Coordinate with services across City Care programs and external service providers to 
• identify appropriate housing candidates, facilitate referrals and required documentation, 
• and support direct placement into City Care Housing when eligibility and program 
• requirements are met.  
•  Collaborate with community partners, including OKC’s Continuum of Care and other 
• service providers, to connect residents with appropriate resources and services.  
•  Assist residents in navigating healthcare, behavioral health services, substance use 
• treatment, public benefits, employment, education, and other community-based systems.  
•  Promote resident independence by encouraging healthy routines, self-advocacy, informed 
• decision-making, personal responsibility, and the development of natural support systems.  
• Assist residents in identifying and addressing barriers that may negatively impact housing 
• stability or quality of life.  
• Maintain accurate and timely documentation of resident contacts, progress, referrals, 
• releases of information (ROIs), service plans, and other required records in accordance 
• with applicable federal, state, local, and program requirements.  
• Provide transportation to scheduled appointments and community resources when 
• appropriate and within program guidelines.  
• Respond to crisis situations using trauma-informed, de-escalation, and person-centered 
• practices.  
• Participate in team meetings, collaborative staffing, and multidisciplinary discussions 
• related to resident needs, progress, and housing stability.  
•  Maintain consistent, meaningful face-to-face engagement with residents at a frequency that 
• exceeds minimum program expectations and supports housing stability, while ensuring all 
• required contacts and service delivery remain compliant with applicable federal, state, 
• local, and program regulations.

•  Demonstrate a commitment to dignity, stability, and opportunity in interactions with 
• residents, colleagues, community partners, and housing providers.    Demonstrate an understanding of the intersection of poverty, homelessness, mental illness, 
• substance use, recovery, physical health, and social isolation.    Ability to reflect recovery principles, with a minimum of three years of sobriety.  
•  Strong written and verbal communication skills.  
• Ability to build genuine rapport while maintaining appropriate professional boundaries.  
• Ability to apply strength-based and person-centered practices that support resident choice, 
• autonomy, and self-determination.   
•  Ability to manage multiple priorities, remain calm under pressure, and maintain accurate 
• and timely documentation.   
•  Ability to work independently while maintaining a high level of accountability.  
• Ability to collaborate effectively with City Care teams, Property Management, and 
• community partners.  
•  Willingness and ability to transport residents in a personal vehicle as required.  
•  Ability to work in environments that may include exposure to poor hygiene, unclean living 
• spaces, and/or bodily fluids.  

Requirements:

•   Previous experience in case management, recovery services, behavioral health, supportive 
• housing, social services, justice services, or related fields.  
•  Previous experience working with adults and/or children living with disabilities.  
•  Ability to use a computer and accurately enter data into an electronic database; HMIS 
• experience preferred.  
•  Successful completion of a pre-employment background check, driving record review, and 
• drug screening.  
• Current Oklahoma driver’s license, reliable transportation, and current automobile 
• insurance. 
•  Previous training or familiarity with de-escalation, trauma-informed care, Housing First, 
• motivational interviewing, and/or person-centered planning.  
• Preferred:  
•   Professional experience navigating mental and physical health systems, public benefits, 
• and community resources.  
•   Previous experience in shelter, supportive housing, or community-based services.  
•  Behavioral Health Case Manager II certification.  
•   Peer Recovery Support Specialist certification, or previous professional and lived 
• experience within a recovery environment.  
•    Knowledge of community resources and service providers throughout the Oklahoma City 
• area.  

Days / Hours/: Full-time, flexible schedule based on resident needs and program operations. 

Schedule may include adjusted daytime and evening hours as necessary to support resident 

engagement, appointments, housing stability needs, and program responsibilities.
