Modivcare is looking for an experienced Facility Outreach Manager to join our team. This position is responsible for serving as an advocate within the non-emergency medical transportation (NEMT) system for patients and case managers by providing in-service training to facilities on how to effectively use NEMT services and promoting strong relationships. This role partners with nursing homes, dialysis clinics, and other medical providers with numerous clients who have scheduled trips to maximize scheduling coordination and information sharing.

This role…

  • Advocates within the non-emergency medical transportation system for patients and case managers.

  • Advocates for the organization’s goals and initiatives within the community.

  • Provides in-service training to facilities on how to use NEMT services and promotes strong relationships.

  • Ensures facility outreach and training are conducted throughout the assigned area of responsibility.

  • Coordinates community-based outreach, event planning, and education.

  • Develops and implements outreach strategies.

  • Develops and maintains ongoing relationships with facilities through face-to-face meetings, phone calls, and group presentations.

  • Analyzes subscription service requests (standing orders), providing education and outreach to prevent or resolve problems and improve quality.

  • Analyzes patient attendance information from facilities for the prior month.

  • Analyzes data and develops cost-management strategies.

  • Oversees facility coordination and quality assurance activities in partnership with the Director of Operations, including staff management.

  • Drives stakeholder engagement.

  • Identifies and contracts with Sole Source TPs, including successful rate negotiation and management of the end-to-end sole source contracting process.

  • Leads facility networking and the tracking and reporting of outreach efforts.

  • Collects and analyzes feedback from the community and stakeholders.

  • Provides training and development related to the TripCare portal.

  • Provides support and guidance to more junior team members.

  • Participates in other projects or performs other duties as assigned.

  • Business travel of up to 80% is required.

We are interested in speaking with individuals with the following…

  • Bachelor’s Degree required.

  • Five (5) plus years of member service, transportation service, UR Manager in a health plan, or social worker experience.

  • Medical transportation management experience preferred.

  • Facility-based social service program experience, including facility discharge planning, preferred.

  • Clinical, case management, and discharge experience preferred.

  • Or equivalent combination of education and/or experience.

  • In-depth knowledge of healthcare facilities, medical groups, clinical case management, and discharge processes.

  • Familiarity with facility-based social service programs, including facility discharge planning.

  • Strong verbal, written, and interpersonal communication skills.

  • Ability to communicate effectively with clients and providers and deliver public presentations.

  • High level of technical competence, including proficiency with Microsoft Office Suite; advanced Excel skills are a plus.

  • Excellent organizational and problem-solving skills, including the ability to develop and implement action plans to address issues.

  • Proactive approach to relationship building.

  • Solutions-oriented mindset with creative and innovative thinking skills.

  • Ability to work independently and as a team member.

  • Strategic thinking skills with the ability to anticipate program, transportation provider, and company needs.

  • Flexibility and ability to manage multiple projects and priorities simultaneously.

  • Knowledge of clinical UR and QA, Medicaid guidelines, and covered services.

  • Knowledge of Medicaid and Medicare programs.

  • Excellent leadership abilities.

Salary: $60,800.00 Annual
