As a Home Health Speech-Language Pathologist, you'll help patients improve communication, cognition, speech, language, and swallowing while restoring confidence and independence in the comfort of their own homes.
You'll have the time to focus on each patient, celebrate progress, and provide care that truly changes lives.

What You'll Do
- Perform comprehensive speech, language, cognitive, and swallowing evaluations
- Develop individualized treatment plans based on patient goals and clinical needs
- Provide skilled speech therapy services in patients' homes
- Educate patients and caregivers on communication strategies, cognitive techniques, and safe swallowing practices
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
Qualifications
- Active Virginia Speech-Language Pathologist License Required
- Certificate of Clinical Competence (CCC-SLP) Preferred
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Communication, and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
