POSITION SUMMARY

The Case Manager is a salaried position stationed primarily in Admissions. The role begins continuing care planning at admission, supports detox-only and direct-admit transitions, coordinates discharge and aftercare, and responds to patients who are considering leaving against medical advice (AMA). The Float Case Manager also serves as a case management resource to the Medical team and, once Admissions responsibilities are current, provides supplemental coverage to Women’s Services first and to other residential programs as directed by the Chief Clinical Officer. The position does not carry a routine permanent caseload.

PRIMARY DUTIES AND RESPONSIBILITIES include some or all of the following:

  1. Admissions and discharge planning. Works primarily from Admissions. Meets newly admitted patients, initiates continuing care planning early, identifies discharge barriers, coordinates aftercare, and ensures continuing care plans (CCPs) are completed or updated and communicated to the receiving program.
  2. Detox and direct admissions. Reviews CCPs for detox-only patients and patients transitioning directly to IOP, Still Waters, or other services. Ensures each plan is accurate, current, and actionable, completes outstanding case management components within scope, and documents a warm handoff.
  3. AMA intervention. Responds to patients expressing intent to leave against medical advice. Provides engagement and practical barrier resolution, coordinates with clinical and medical teams, and documents interventions and outcomes according to policy.
  4. Medical team support. Serves as a case management resource for Medical, particularly when patients need discharge coordination, aftercare arrangements, or assistance resolving barriers.
  5. Women’s Services coverage. When Admissions responsibilities are current, provides first-priority supplemental case management support to Women’s Services, including CCP follow-up, discharge tasks, and temporary workload support.
  6. Other program coverage. Provides additional case management assistance across residential programs based on operational needs and at the direction of the Chief Clinical Officer.
  7. Weekend coverage. Works Sundays as part of the regular schedule, providing case management coverage for weekend admissions, AMA response, and preparation of continuing care and discharge arrangements for patients discharging early in the week.
  8. Discharge coordination. Secures aftercare appointments, verifies referrals, and arranges transportation, housing, or recovery supports as needed; identifies and escalates issues that could compromise a safe transition.
  9. Treatment team collaboration. Works with primary counselors and treatment teams on treatment plan and biopsychosocial information as appropriate to role qualifications, without assuming primary counselor responsibilities.
  10. Care coordination. Provides timely documentation and information to Utilization Review, Financial Counseling, Admissions, and approved referral sources to support continuity of care.
  11. Documentation. Maintains accurate electronic health record documentation and case management tracking of CCP status, discharge barriers, AMA contacts, referrals, and handoff ownership.
  12. Communication and improvement. Communicates clearly across shifts and departments, promptly escalates patient safety and continuity concerns, and recommends improvements to admissions-to-discharge workflows.
