Description

This is a grant-funded position supported by Oklahoma Complete Health.

About Northside Neighbors

Launched through the InvestNorth Tulsa initiative, Northside Neighbors is a resident-driven nonprofit organization dedicated to fostering opportunity, well-being, and prosperity in North Tulsa. Our work is centered on four interconnected areas: Mixed-Income Housing, Cradle-to-Career Education, Community Wellness, and Economic Vitality.

As a member of the Purpose Built Communities Network, Northside Neighbors uses a holistic, resident-centered approach to neighborhood revitalization. We work alongside residents, community organizations, institutions, businesses, and other partners to strengthen neighborhoods and create conditions for individuals and families to thrive.

Resident voice and engagement are foundational to our work, and Northside Neighbors serves as a community quarterback—helping connect people, resources, organizations, and strategies around shared neighborhood priorities.

Position Purpose

The Community Health Prevention and Promotion Manager is responsible for leading the development and implementation of a Health Empowerment Zone serving the Hawthorne and Walt Whitman neighborhoods and greater North Tulsa. A Health Empowerment Zone (HEZ) is a place-based, community-led approach to improving health by addressing the social, economic, and environmental conditions that shape people’s health before they see a doctor

Working closely with the Director of Community Wellbeing, the Manager will lead community-based health promotion and prevention strategies, strengthen partnerships, increase connections to primary care and mental health services, and coordinate key initiatives including the “Know Your Numbers” campaign, expanded doula training and services, and two annual Community Baby Showers. The position will ensure that resident voice and community priorities guide the work and that health strategies are integrated across Northside Neighbors' broader neighborhood revitalization efforts.

Responsibilities and Essential Duties

Health Empowerment Zone Development and Implementation

  • Lead the development, implementation, and ongoing coordination of the Northside Neighbors Health Empowerment Zone.
  • Develop annual workplans, goals, strategies, timelines, and measurable outcomes for Health Empowerment Zone activities.
  • Identify community health priorities through resident engagement, available health data, partner input, and community assessments.
  • Coordinate prevention and health-promotion strategies that respond to the needs and priorities of residents in Hawthorne, Walt Whitman, and greater North Tulsa.
  • Ensure Health Empowerment Zone activities are aligned with Northside Neighbors' strategies related to Resident Voice, Community Quarterback, Mixed-Income Housing, Cradle-to-Career, Community Wellbeing, and Economic Vitality.
  • Identify opportunities to address social, economic, environmental, and neighborhood conditions that affect health and well-being.

Health Promotion and Prevention Initiatives

  • Develop and coordinate the “Know Your Numbers” campaign to increase community access to biometric screenings, preventive health information, and appropriate follow-up care.
  • Build partnerships with healthcare organizations and qualified providers to offer screenings and health education at community events and neighborhood locations.
  • Develop culturally relevant health education and outreach strategies that encourage preventive care and increase residents' understanding of important health indicators.
  • Coordinate community health education activities, workshops, campaigns, and events based on identified neighborhood health priorities.
  • Promote opportunities for residents to establish relationships with primary care providers and access appropriate preventive services.

Maternal and Infant Health

  • Partner with the Oklahoma Birth Equity Initiative and other maternal health organizations to expand access to doula training, education, and services in North Tulsa.
  • Support the development and expansion of a network of doulas and maternal health resources serving North Tulsa families.
  • Coordinate the planning and implementation of two Community Baby Showers annually in North Tulsa.
  • Build partnerships with healthcare providers, community organizations, maternal health professionals, and service agencies to connect expectant parents and families with resources.
  • Increase awareness of prenatal, postpartum, maternal, infant, and family support services available throughout North Tulsa.

Access to Primary Care and Mental Health Services

  • Develop strategies to increase residents' awareness and utilization of primary care, behavioral health, and mental health services.
  • Strengthen referral pathways between residents, healthcare providers, community-based organizations, and social service partners.
  • Promote healthcare and supportive services available along the 36th Street North corridor and throughout North Tulsa.
  • Work with healthcare partners to identify and reduce barriers that prevent residents from accessing care.
  • Coordinate with Community Health Workers and other frontline staff to help connect residents to appropriate health and social services.
  • Identify gaps in available healthcare resources and work with partners to develop strategies for addressing those gaps.

Resident and Community Engagement

  • Ensure resident voice and lived experience are central to the planning, implementation, and evaluation of Health Empowerment Zone activities.
  • Build trusted relationships with residents of Hawthorne and Walt Whitman and the broader North Tulsa community.
  • Conduct and participate in community meetings, listening sessions, focus groups, outreach activities, health events, and other resident engagement opportunities.
  • Recruit residents to participate in health initiatives, advisory groups, community events, and other Health Empowerment Zone activities.
  • Develop outreach strategies that reach residents who may experience barriers to accessing traditional healthcare systems.
  • Collaborate with faith communities, schools, neighborhood organizations, healthcare providers, businesses, and other trusted community institutions.

Partnership Development and Coordination

  • Develop and maintain strong working relationships with hospitals, clinics, healthcare providers, public health organizations, behavioral health providers, community organizations, funders, and other partners.
  • Convene partners around shared community health priorities and coordinate collaborative strategies when appropriate.
  • Represent Northside Neighbors at community health meetings, coalitions, workgroups, conferences, and partner meetings.
  • Identify opportunities to leverage partner resources, programs, expertise, and funding to expand health-related services available to North Tulsa residents.
  • Maintain regular communication with partners and ensure timely follow-up on commitments and collaborative activities.

Data, Evaluation, and Grant Management

  • Track Health Empowerment Zone activities, participation, referrals, partnerships, outputs, and outcomes.
  • Establish and maintain systems for collecting accurate program and community health data.
  • Use data and resident feedback to evaluate program effectiveness, identify gaps, and make recommendations for continuous improvement.
  • Monitor progress toward grant goals, deliverables, timelines, and performance measures.
  • Prepare required program reports, grant reports, presentations, summaries, and other documentation.
  • Maintain accurate records and supporting documentation for grant-funded activities and expenditures.
  • Work with organizational leadership to communicate program accomplishments, challenges, community impact, and emerging health needs.
  • Support the identification of additional funding opportunities to sustain and expand successful Health Empowerment Zone strategies.

Organizational Responsibilities

  • Work closely with the Director of Community Wellbeing and collaborate with other Northside Neighbors staff to ensure health initiatives are integrated across organizational strategies.
  • Participate in staff meetings, strategic planning, community events, and organizational initiatives.
  • Represent Northside Neighbors professionally with residents, partners, funders, and the broader community.
  • Maintain confidentiality and appropriately handle sensitive resident, health, program, and organizational information.
  • Participate in occasional evening and weekend activities related to community events and programs.
  • Perform other duties as assigned in support of Northside Neighbors' mission.
