Setting: Home-based care / mobile healthcare model providing patient-centered care directly in patients’ homes and through telehealth

Schedule: Full-time

Hours: Flexible schedule based on patient needs and home visits

Job Requirements: Active Florida RN and APRN license, Master’s or Post-Master’s degree from an accredited APRN program, ability to meet Florida protocol requirements, minimum of 3 years of APRN experience preferred, experience caring for adult and geriatric populations preferred, strong clinical assessment and diagnostic skills, knowledge of ICD-10 and CPT coding, EMR experience (Athena preferred), excellent communication and patient education skills, ability to work independently while collaborating with a multidisciplinary healthcare team, reliable transportation, and willingness to travel throughout Palm Beach and Broward Counties
