Job Description:

The Care Coordination Operations Manager provides operational leadership for care coordination services supporting proactive care, population health, and affiliate practice initiatives. This role is responsible for the day-to-day operations, performance, staffing, and continuous improvement of care coordination teams. The Manager partners with clinical, operational, and analytics stakeholders to ensure care coordination services are delivered efficiently, consistently, and in alignment with organizational quality, patient experience, and value-based care objectives.

The Care Coordination Operations Manager leads supervisors and frontline teams, establishes performance expectations, develops operational workflows, monitors outcomes, and drives accountability for achieving departmental goals. This leader supports the implementation of strategic initiatives while ensuring standardized execution of care coordination programs across assigned areas. The role collaborates with Network Operations, Care Management, Medical Group leadership, Analytics, and affiliate practice stakeholders to improve patient outcomes and operational performance.

Essential Functions

  • Provides operational leadership and oversight for care coordination services, ensuring effective daily operations, staffing, workflow execution, and service delivery.
  • Directs, coaches, and develops supervisor-level leaders through performance management, accountability, succession planning, and leadership development activities.
  • Establishes and monitors productivity, quality, service, and operational performance metrics; uses data to identify opportunities and implement improvements.
  • Partners with clinical, operational, and physician leadership to align care coordination services with organizational priorities, patient care needs, and population health goals.
  • Leads implementation and sustainment of standardized workflows, operational best practices, and continuous improvement initiatives.
  • Manages staffing, resource allocation, workload distribution, and operational capacity planning to ensure effective team performance and service coverage.
  • Identifies operational barriers, process variation, and performance risks; develops and implements solutions that improve efficiency, caregiver experience, and patient outcomes.
  • Leads operational change initiatives, including implementation of new technologies, workflows, programs, and operational processes.

Minimum Qualifications

  • Demonstrated three years of experience in healthcare operations, care coordination, care management, population health, clinical operations, practice transformation, or a related healthcare operations or leadership environment.
  • Demonstrated experience leading teams, projects, operational initiatives, or workflow improvement efforts.
  • Experience utilizing operational metrics, reports, and performance data to guide decision-making and improve outcomes.
  • Strong verbal and written communication skills.
  • Strong computer skills, including Microsoft Office applications and other applicable operational systems.

Preferred Qualifications

  • Associate’s degree or equivalent combination of education and relevant healthcare operations, care coordination, population health, or leadership experience.
  • Leadership experience managing supervisors or other people leaders.
  • Experience in care coordination, care management, population health, value-based care, ambulatory operations, or patient outreach programs.
  • Experience leading process improvement and operational transformation initiatives.
  • Experience with Lean principles, continuous improvement methodologies, or workflow standardization.
  • Experience collaborating with physicians, clinical leaders, operations leaders, analytics teams, and cross-functional stakeholders.

Physical Requirements

  • Ongoing need for employees to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs. 
  • Frequent interactions with providers, colleagues, customers, patients/clients, and visitors require employees to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately. 
  • Manual dexterity of hands and fingers to manipulate equipment and perform frequent computer work, including typing and accessing information.
  • Operate computers, telephones, and office equipment.
  • Remain seated and/or standing for extended periods while performing work activities.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$29.58 - $45.66
