Monday through Friday 8:00am-4:30pm with varying Saturdays.
Pay Range:
$21.85 - $32.80Engages a population of socially vulnerable patients in their care, assist them through the process of working towards better health by providing support, encouragement, and education in person, by telephone and via electronic means.
Builds relationship with respect to diversity, using active listening, casual counseling to strengthen the patient’s health, social and personal goals.
CHW will operationalize the care coordinator’s care plan by providing on-going support to the patients, educate them on self-management of their health conditions, proper use of the emergency room, help patients to overcome barriers to obtaining medical care/social, access community resources, etc.
Act as a patient advocate and liaison between the patient/family and community service agencies (i.e., schools, DHS, hospital, etc.).
Education Required:
High School diploma or GED required.
Preferred Education
Bachelor’s degree in a relevant human service field
Certification Required:
Valid GA driver’s license required, excellent driving required and have reliable transportation.
