Duties:
Licensing Standards-
- The Designated Coordinator must provide quality supervision, support, and evaluation of activities that include:
- Maintains and performs periodic audits of Individual's file
- Maintains and summarizes data for reports on the outcomes of services for support team members
- Monitors the availability of meals three times daily and ensures sanitary preparation
- Monitors the program site including, maintenance, cleaning, utilities, adequate furniture and furnishings, and neighborhood relations
- Trains all employees per site training requirements of the Individual's plan within the required timelines
- On call as needed per group needs
Healthcare-
- Administers of medical or health related services including proper documentation of all prescribed medication, treatments, diets and or exams.
- Coordinates appointments for all health or consultant related services ensuring resolution of outstanding issues.
- Manager contracted health services as assigned for Individuals served ensuring effective and timely delivery
Financial Management-
- Manages Individual funds and needs based on authorization from service team
- Administers the financial resources of the program and petty cash accounts
Employee Relations -
- Works with HR/Training Team to ensure all staff maintain mandated personnel, training, and 245D requirements
- Provides quality training for program and Individual needs
- Provides timely and constructive feedback to Employees on-going performance
- Coordinates staff scheduling based on support ratio requirements
- Effective management of Employee Pavillio and payroll requirements daily
- Efficiently and effectively communicates and collaborates with teams
Minimum Qualifications:
- A positive attitude and willingness to learn
- Compassion and a desire to help others
- 18 years of age or older
- Ability to pass a background check
- Valid US driver's license
- Basic computer skills
- Ability to read, write, and speak the English language
Qualifications:
A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to people with disabilities or people age 65 and older;
An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to people with disabilities or people age 65 and older;
A diploma in the field of human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to people with disabilities or people age 65 and older; or
A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to people with disabilities or people age 65 and older under the supervision of a staff person
