What You'll Do

As a QIDP, you will coordinate and oversee services for individuals receiving residential and day supports while ensuring services remain person-centered, compliant, and focused on positive outcomes.

Every day, you'll help individuals build independence, achieve personal goals, strengthen community connections, and live the lives they choose.

Key Responsibilities

• Coordinate and monitor person-centered services and supports
• Facilitate and oversee Comprehensive Functional Assessments (CFAs) within Intermediate Care Facilities (ICFs)
• Collaborate with interdisciplinary teams to develop, implement, and monitor individualized service plans
• Provide case management and service coordination for individuals receiving supports
• Ensure compliance with regulatory requirements, Medicaid standards, and agency policies
• Maintain accurate documentation, records, and reports
• Monitor program quality and advocate for the needs and goals of individuals served
• Provide leadership, guidance, and support to team members
• Communicate effectively with individuals served, families, staff, and community providers
• Identify service needs and implement proactive solutions to support positive outcomes

Preferred Experience

• Experience working in residential, day habilitation, or community-based support programs
• Experience with person-centered planning and service coordination
• Background in Social Work, Human Services, Psychology, or related disciplines
