Summary

JOB SUMMARY:Reporting to the Patient Care Manager and taking direction from the Clinic Coordinator, the Community Health Nurse, assesses, identifies, plans, implements and evaluates the nursing care required to assist clients with complex health care needs in meeting their physical, social, spiritual and psychological goals using an established framework for nursing practice. Develops, implements, evaluates and revises the plan of care and supports the transition of care. Incorporates individual and/or group teaching into the care plan and nursing interventions and/or makes referrals to community resources/ services and other identified resources. Supports the interdisciplinary team by collegially sharing information and demonstrating procedures. The Community Health Youth Clinic Nurse works within an interdisciplinary clinical framework in accordance with the BCCNM Standards and consistent with the mission, vision and values and established protocols of Providence Health Care. QUALIFICATIONS:Education, Training and ExperienceCurrent practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) plus two (2) years’ recent, related experience working with youth and young adults in a primary care and mental health/substance use setting; completion of the Point of Care HIV Testing course[AM1.1]; and successful completion of the BCCDC Immunization certificate. BCCNM Certified practiced in Reproductive Health (Sexually Transmitted Infections (STI)) or evidence of enrollment in required certified practice courses as specified by the BCCNM.Skills and Abilities• Knowledge of and exposure to trauma-informed care, violence prevention, gender-affirming care, harm reduction, recovery-oriented practice and indigenous cultural safety training. • Demonstrated knowledge of wound care, addiction assessments, harm reduction, HIV and HCV testing, phlebotomy and pelvic exams, diabetes care, pain management, STI screening, testing, diagnosing and treatment within scope of nursing certified STI practice, and pregnancy and urine drug tests. • Demonstrated knowledge of client/patient-centered education principles, methods and tools as it relates to healthcare needs including pain, addiction, diabetes, wound care. • Ability to role model and employ effective conflict resolution, de-escalation and reconciliation approaches and techniques. • Ability to communicate effectively both verbally and in writing.• Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.• Ability to work as a member in an interdisciplinary team.• Ability to apply decision making and analytical skills.• Ability to deal with others effectively.• Ability to role model conflict resolution strategies.• Physical ability to carry out the duties of the position.• Ability to prioritize and organize work.• Ability to operate related equipment.• Ability to teach and identify client learning needs.• Ability to lead patient/family group sessions.• Ability to provide clinical direction.Reviews client status through methods such as reviewing the client chart and other written information, client observation, interviewing client and family members, completion of questionnaires, collateral information and consultation with peers, community resources/ services and other members of the interdisciplinary care team. Assesses client problems and needs using health management and/or self-care management, plans, implements, and prioritizes nursing care and nursing interventions and develops the plan of care for clients using clinical judgement and evidence-based practice. Provides outreach services in the community when required.Performs primary care nursing duties such as wound care, addiction assessments, harm reduction, HIV and HCV testing, phlebotomy and pelvic exams, diabetes care, pain management, STI screening, testing, diagnosis and treatment within scope of nursing certified STI practice, and pregnancy and urine drug tests according to identified policies and procedures, technology and PHC standards. Monitors client progress, evaluates outcomes based on client goals, reassesses if goals are not met, and modifies plan of care based on evaluation and supports the transition. Records observations and reports problems and/or changes in client condition to designated staff.Provides direct nursing interventions for management of crisis and life threatening situations. Documents information including assessments, plan of care, client status and progress and transitions of care or discharge plans in a manual and/or computerized environment.Acts as a resource for the primary care team by providing information based on nursing theory and practice related to clients with complex health care needs, collegially shares information, provides demonstrations and/or guidance on related tasks. Identifies and responds to client and family learning needs, provides client educational and reference materials, incorporates individual and/or group teaching activities into the care plan and nursing interventions, and/or makes referrals to community resources/ services or other identified resources. Works collaboratively with interdisciplinary care team to facilitate transitions in care using the integrated step care model or discharge planning and liaises with families and community resources/ services agencies.Participates in nursing orientation and shares clinical knowledge with other nursing staff, nursing students and other members of the interdisciplinary team. Participates in and attends in-service presentations and other educational programs as required. Participates in planning, implementation and evaluation of client care policies, procedures, education materials, and best practices through methods such as taking part in program and quality improvement activities including nursing practice council, clinical pathway development, research and education and other committees. Supports nursing research through methods such as identifying areas for possible clinical nursing research projects, adapting research to practice and participating in research activities to enhance nursing practice. Advocates for the clients, to protect and promote their rights to dignity, respect, privacy, confidentiality, autonomy and access to information.Performs other related duties as assigned.
