Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Physical Therapist (PT) for Element Care PACE in Brighton, Massachusetts.
POSITION OVERVIEW
• Provide comprehensive physical therapy evaluations, treatment planning, therapeutic interventions, and home safety assessments for older adults enrolled in the PACE program
• Deliver care in both the Adult Day Health Center and participants' homes throughout the service area
• Develop individualized plans of care focused on maintaining independence and preventing functional decline
• Supervise Physical Therapist Assistants (PTAs) and rehabilitation aides as needed
• Collaborate with an interdisciplinary PACE team while maintaining autonomy in clinical decision-making
COMPENSATION
• $84,000-$113,000 Annual Salary
• Mileage Reimbursement
SCHEDULE
• Monday-Friday
• 8:00 AM-4:00 PM
• Approximately 6 Patients Per Day
• No Evenings
• No Weekends
• No On-Call Responsibilities

Requirements
• Active Massachusetts Physical Therapist License REQUIRED
• Minimum One Year of Physical Therapy Experience with Older Adults REQUIRED
• Experience Providing Care in Clinical and Home-Based Settings Preferred
• Experience Completing Evaluations, Treatment Plans, and Home Safety Assessments REQUIRED
• Ability to Supervise Physical Therapist Assistants (PTAs) and Rehabilitation Aides
• Strong Communication and Interdisciplinary Team Collaboration Skills
