Overview
As a Home Health RN Case Manager, you will administer skilled nursing care to patients on an intermittent basis in their place of residence. You'll perform initial evaluations, develop care plans, coordinate with interdisciplinary teams, and maintain patient records. Requires graduation from an approved nursing school, current RN license, and one year of nursing experience preferred. Must demonstrate initiative, independent judgment, and maintain patient confidentiality.
