Hospital Social Worker

We are working closely alongside a Local Authority in Lambeth to assist with the appointment of a Hospital Social Worker, on a 6-months contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration.

Rate of Pay: £28.56 - £38.87 per hour

Summary:

To provide high-quality statutory social work services to adults aged 18 and over who are admitted to King's College Hospital and other hospitals outside the borough, ensuring that their care, support, safeguarding and discharge needs are assessed and appropriately addressed.

Responsibilities:

• Undertake comprehensive Care Act assessments of adults with care and support needs.
• Assess the needs of individuals with physical disabilities, long-term conditions, vulnerabilities and complex care requirements.
• Identify individual strengths, abilities, resources and informal support networks as part of a strengths-based assessment.
• Review existing care and support arrangements and amend services where circumstances or needs have changed.
• Ensure assessments and care plans are completed accurately and within statutory and service timescales.

Qualifications

• Recognised professional qualification in Social Work.
• Current registration with Social Work England or any successor regulatory body.
• Successful completion of the ASYE programme, where applicable, or sufficient statutory social work experience as agreed by the Principal Social Worker.

Essentials

• Significant experience working as a qualified Social Worker within a statutory adult social care environment.
• Experience undertaking Care Act assessments for adults with care and support needs.
• Experience developing, implementing and reviewing person-centred care and support plans.
• Experience working with adults with physical disabilities, health conditions, vulnerabilities or complex care needs.

• You should be available to work immediately or at a short notice.
• You should have right to work in U.K
• This role requires an Enhanced DBS
