Job Summary
Join our growing Home Health team as a Start of Care Registered Nurse (SOC RN). In this role, you will complete comprehensive patient admissions, perform OASIS assessments, develop individualized care plans, and coordinate home health services to ensure quality patient outcomes and regulatory compliance.
Key Responsibilities
- Conduct Start of Care (SOC) and admission assessments in patients' homes.
- Complete accurate OASIS documentation and clinical records.
- Develop and implement individualized care plans.
- Educate patients and caregivers on disease management, medications, and home safety.
- Coordinate care with physicians, therapists, and the interdisciplinary team.
- Identify patient needs, arrange services and equipment, and initiate discharge planning.

What We’re Looking For
- Current RN license in the State of Arizona.
- Fingerprint Clearance Card.
- Valid driver’s license and reliable, insured vehicle.
- Strong clinical skills and a passion for patient-centered care.
- Ability to work independently and make a real difference.
