As a Clinical Coordinator with Salvos Home Care, you’ll play a key role in delivering high-quality, person-centred care under the evolving Support at Home program - helping older Australians remain independent, safe, and well in their own homes.

ABOUT THE ROLE
In this dynamic, community-based role, you’ll lead the clinical coordination and management of services for a portfolio of Home Care clients. Working closely with clients, families, and internal teams, you’ll ensure care delivery aligns with Support at Home principles, focusing on independence, reablement, and tailored clinical support.
This role involves regular travel across the Central Coast, with visits scheduled based on client needs.
You will:
- Conduct clinical assessments, onboarding, and ongoing reviews for Home Care clients
- Developing, reviewing and monitoring personalised care plans and quarterly budgets
- Provide clinical oversight and respond proactively to changing care needs
- Deliver hands-on care where required, including wound care, medication management, PAS assessments and health monitoring
- Support clients with more complex or higher-level clinical needs in collaboration with the broader team
- Identify risks and implement timely clinical interventions and referrals
- Mentor and support Home Care Workers, promoting safe, high-quality care delivery
ABOUT YOU
- Registered Nurse with current AHPRA registration
- Clinical experience in community or aged care
- Strong leadership skills with the ability to mentor and support care teams
- Understanding of consumer-directed care and Support at Home principles
- Excellent communication and relationship-building skills
- Strong clinical assessment and organisational abilities
- Current driver’s licence and willingness to travel
- Alignment with and willingness to support the Christian values and ethos of The Salvation Army
