We're seeking a compassionate Occupational Therapist to provide one-on-one home health services in the Cherry Hill area, helping older adults improve function, confidence, and independence.

What You'll Do:
- Perform occupational therapy evaluations in patients' homes.
- Design individualized treatment plans based on each patient's functional goals.
- Address limitations affecting self-care, mobility, and everyday activities.
- Recommend adaptive equipment, home modifications, and safety strategies that support independent living.
- Educate patients, family members, and caregivers on techniques that promote long-term success.
- Monitor patient progress and update treatment plans as needed.
- Complete timely electronic documentation in accordance with Medicare and agency guidelines.

Qualifications:
- Current NJ State Occupational Therapist license.
- Strong clinical reasoning and communication skills.
- Ability to work independently in a community-based setting.
- Experience with adult or geriatric patients is preferred but not required.
- Home health experience is welcome but not necessary.
