Department & Position Overview:
FreedomCare’s Missouri operations support In-Home Services (IHS), a Medicaid home and community-based services model that allows eligible participants to receive in-home assistance to help them live independently.
The Care Manager, LPN is responsible for conducting initial in-home assessments, annual home visits, and providing ongoing care management and support. This role combines clinical assessment, care coordination, patient and caregiver education, and quality assurance to ensure safe, effective, and person-centered care.
The Care Manager, LPN collaborates closely with patients, caregivers, and interdisciplinary care teams to assess needs, identify potential changes to the individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the LPN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance. The Care Manager, LPN proactively communicates clinical concerns, changes in condition, and potential barriers to care with the appropriate team members to ensure timely intervention, effective care coordination, and optimal patient outcomes.
Key Responsibilities:
- Conduct initial in-person clinical assessments and coordinate timely service activation, ensuring all required documentation, approvals, and standard procedures are completed.
- Identify potential changes to the individualized plans of care, making and documenting revisions as the patient’s needs or condition change.
- Educate patients, caregivers, and internal teams regarding care needs, safety considerations, service requirements, caregiver responsibilities, and next steps.
- Coordinate patient care through timely communication and documentation with physicians, clinical disciplines, internal partners, and appropriate community resources.
- Provide ongoing education and coaching to support effective use of established best practices, promote caregiver competency, and connect patients and families with appropriate resources.
- Review participant service records regularly and respond appropriately to identified changes in the patient’s condition, environment, or service needs.
- Identify changes in condition, safety concerns, service gaps, incidents, or other urgent issues and escalate them in accordance with established clinical protocols.
- Complete and document assessments, visits, clinical observations, care-plan updates, incidents, communications, and follow-up actions accurately, thoroughly, and within required timeframes.
- Support authorization, renewal, start-of-care, and continued-service processes, as applicable.
- Maintain compliance with all applicable regulatory requirements, professional standards, and company policies and procedures.
- Perform other related duties as assigned by the supervisor.
Credentials:
- Diploma from an accredited nursing program.
- Valid Licensed Practical Nurse license and in good standing by Missouri Board of Nursing
- Minimum of (1) year of experience with the care of the elderly, or individuals with disabilities or medically complex conditions.
- Valid driver’s license and car insurance required, with means of reliable transportation.

- Effective verbal and written communication skills, with the ability to clearly summarize interactions, document case activity, and communicate case status and next steps.
- Strong typing and computer proficiency, with the ability to efficiently navigate contact center technology, customer relationship management systems, telephone platforms, and other necessary software applications while accurately documenting interactions.
- Effective problem-solving and critical thinking, with the ability to evaluate information, identify appropriate solutions, exercise sound judgment, and escalate concerns when necessary.
- Dependable and punctual, with a demonstrated commitment to consistent attendance, schedule adherence, and performance expectations.
- Ability to adapt to changing priorities, processes, systems, and business needs in a fast-paced environment.
- Receptive to coaching and feedback, with the ability to apply guidance and demonstrate continuous improvement.
- Ability to collaborate effectively as part of a team while working independently within established guidelines.

At FreedomCare, base pay is one part of our total compensation package and is determined within a range. This provides our employees with the opportunity to professionally grow and develop within a role. The base pay range for this role is between $30.00 and $40.00 an hour at the commencement of your first year of employment. Compensation decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
