Position Summary / Career Interest:

The Care Coordination Specialist (CCS) supports Medicaid Incentive Payment (MIP) Program implementation across primary care practices in rural Kansas communities. This position plays a key role in use of the electronic health record for identification of eligible patients, clinical workflows for successful patient engagement, coordination with Medicaid Managed Care Organizations for referral of patients, documentation and submission of incentive payments, and helping facilities adopt and sustain systemwide best practices.

The Care Coordination Specialist will support the Rural Health Transformation Program goal to improve Chronic Care Management adoption for Medicaid patients, address gaps in MCO performance, and coordinate with related initiatives including dual-eligible and remote patient monitoring programs.

This position provides expertise in change management, electronic health records (EHR) registry development, patient engagement strategies, and performance reporting – ensuring accurate data capture, registry functionality, and actionable reporting for both clinical and operational leaders.

Responsibilities and Essential Job Functions
  • Guide facilities through the implementation and system-defined best practices.
  • Match patients to the right services (MCO CCM, organization processes, Patterson-funded support, or RPM/ambient tools if dual-eligible, or high-risk)
  • Identify dual eligible for ambient listening/remote monitoring equipment deployment to prevent nursing facility placement
  • Coordinate with FQHCs via Community Care Network of Kansas (ACO context) and RHCs
  • Support high-risk maternity CCM where relevant
  • Embed processes in clinic workflows for identifying eligible Medicaid patients (via EHR reports or Medicaid data lists)
  • Support patient consent
  • Validate clinical compliance, documentation accuracy, and process adoption.
  • Coach rural teams, clinical champions, and operational leaders on MIP expectations and best practices.
  • Generate, interpret, and distribute timely reports for facilities and system leadership.
  • Establish incentive payment workflows and processes.
  • Build strong, productive relationships with hospital leadership, clinic managers, quality leaders, and frontline teams.
  • Participate in system committees, collaborative groups, and project teams focused on improvement and standardization.
  • Extensive travel (up to 70%), including overnight stays and travel to rural clinics and community hospitals.
  • Ability to work independently while traveling with minimal on-site supervision.
  • Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.

Required Education and Experience
  • Bachelors Degree in health care, business administration, or related field
  • Significant knowledge and experience in primary care clinic operations

Preferred Education and Experience
  • Master's Degree
  • 5 or more years of healthcare experience within ambulatory practices

Required Licensure and Certification
  • Valid, unrestricted driver’s license, Kansas Class C or equivalent. MVR results must be in compliance with Health System Safe Driving Program Policy. Candidate must have access to reliable, personal transportation. Vehicle must carry insurance with at least the minimum automobile liability insurance levels required by the state(s) of operation.

Preferred Licensure and Certification
  • Licensed Registered Nurse (LRN) - Single State - State Board of Nursing

Knowledge Requirements
  • Proficiency in Microsoft Office and Teams
  • Baseline knowledge in Medicaid and Medicare reimbursement
