Clinic Care Coordinator LPN/RN (FM)

MANA Family Medicine Southwest Bentonville is seeking a compassionate, proactive, and patient-focused Clinic Care Coordinator to help improve the health and well-being of our patients through personalized care coordination and population health initiatives.

 

In this role, you'll serve as a key connection point between patients, providers, clinic staff, and community resources. You'll help patients navigate their healthcare journey, ensure they receive the preventive and follow-up care they need, and support those who may be at greater risk for health complications.

 

As part of our value-based care efforts, you'll work closely with providers and clinic teams to improve quality outcomes, enhance patient engagement, and promote exceptional, coordinated care for the people of Northwest Arkansas.

 

What You'll Do

• Coordinate care and support patients in achieving their healthcare goals by partnering with providers, specialists, community resources, and health plans.
• Conduct patient outreach to help patients stay engaged in their care, complete recommended screenings, and receive timely follow-up services.
• Review patient records and identify opportunities to close care gaps, improve outcomes, and support preventive healthcare initiatives.
• Connect patients with internal and external resources, including specialty care, disease management programs, home health services, and community support organizations.
• Collaborate with providers and clinic teams to improve quality outcomes, patient experience, and value-based care performance.
• Assess barriers to care, including social determinants of health, and help develop personalized plans that support each patient's wellness and independence.
• Build relationships with local healthcare partners and care teams to ensure seamless coordination across the continuum of care.
• Educate providers and staff on quality measures, documentation best practices, and population health initiatives.
• Analyze patient population trends and quality data to identify opportunities for improvement and develop action plans that support better health outcomes.
• Serve as a trusted resource for patients and care teams while helping advance MANA's mission of delivering exceptional, coordinated care.

Qualifications: 

• LPN or RN active license
• 12-18 months related experience and/or training
• Prior Family Medicine experience preferred, but not required
