Admissions Nurse – Part-Time

Schedule: Fridays, Saturdays, and Sundays
Pay Range: $31.00–$32.00 per hour, based on experience

San Joaquin Nursing and Rehabilitation is seeking a compassionate, organized, and dependable Admissions Nurse to join our team on a part-time basis. The Admissions Nurse will help ensure a smooth and welcoming transition for new residents while completing all required clinical assessments, documentation, and admission procedures.

Key Responsibilities

• Coordinate and complete nursing portions of new resident admissions
• Perform initial nursing assessments and document residents’ conditions
• Review hospital records, physician orders, medications, diagnoses, and treatment plans
• Verify that admission documentation is complete, accurate, and entered promptly
• Complete medication reconciliation and communicate discrepancies to the appropriate provider
• Ensure residents and families receive a welcoming and informative admission experience
• Communicate clinical needs and changes in condition to physicians and nursing leadership
• Collaborate with nursing, therapy, dietary, social services, and other departments
• Assist with developing initial care plans based on residents’ individual needs
• Follow infection-control practices and all applicable facility, state, and federal requirements
• Maintain the confidentiality of resident health information

Qualifications

• Current California nursing license in good standing
• Current CPR/BLS certification
• Previous admissions experience in a skilled nursing or long-term care setting preferred
• Strong clinical assessment, communication, and documentation skills
• Knowledge of medication reconciliation and admission procedures
• Ability to work independently and manage multiple priorities
• Compassionate, reliable, and committed to excellent resident care
• Must be available to work Fridays, Saturdays, and Sundays
