Position Overview:

Designs and implements the strategic and ongoing development in a defined geographic area to provide health care programs and other services to the vulnerable populations as identified through the operating units Community Health Needs Assessment. Manages multiple projects and resources to include leading and securing funds for community health programs/initiatives, and coordinating and managing all community benefit activities in alignment with system and operating unit goals, and the system External Affairs organization in a defined geographic area that advance the Sutter's community based, not-for-profit mission.

Develops and maintains relationships with key community and business leaders, across various communities and issue areas, and identifies potential collaborative partners/efforts aimed to positively impact the community. Identifies, plans, coordinates, implements, monitors and evaluates effective community programs and direct service partnerships that aim to improve community health, support the under-served and advance Sutter Health's reputation in the community and not-for-profit mission.

Manages the active engagement of program implementation, assists with budget development, as well as program evaluation within the assigned geography. Manages a team of Coordinators and/or Analyst in the delivery of the programs in the assigned geography.

JOB ACCOUNTABILITIES:

Strategy & Program Deployment
• Helps lead, design and implement programs or initiatives based on the identified needs of the community.
• Helps develop and oversee implementation of the affiliates Community Benefits Plans, as mandated by Senate Bill 697 (SB 697) and IRS.
• Manages the creation, distribution and implementation of the tri-annual Community Health Needs Assessment for defined geographic areas or affiliates.
• Assists in the determination of Sutter Health’s participation in relevant and essential community collaborative efforts as well as in the exploration of new opportunities for OU leadership involvement.
• Helps develop and implement OU Wide Health Initiatives on behalf of operating unit affiliates.
• Provides input and strategic design around Charity Care, Medi-Cal and health access program partnerships.
• Implements and manages direct service community health programs and clinics.
• Works with leadership team and key constituents regarding programs within affiliates to analyze existing community benefit activity and its effectiveness.
• Makes recommendations on programs and allocation of resources to maximize efficacy of community benefit activity.
• Understands the political climate and the need for relationship management to inform community and municipal public health stakeholders of affiliate programs and services. Assists in development of an organization plan for external audiences.
• Assists in the development of an annual operating unit Strategic Community Health Plan.
• Assists in development and implements a scope of work for the operating unit Community Needs Assessment in collaboration with the region’s other health systems.
• Identifies opportunities for collaboration with community-based organizations to improve community health and advance Sutter Health’s position in the community.
• Develops and oversees strategy around strengthening the network of Federally Qualified Health Centers and community clinics in the operating unit service area.
• Works across the entire Sutter Health network to develop and expand best practices and streamline process.
• Ensures coordination of Community Benefit Planning with Strategic planning process through collaboration with the Director of Community Health.
• Manages the development of the OU’s annual Strategic Community Benefit Plan for defined geographic areas or affiliates.
• Manages OU community benefit activities including events, programs, grants and sponsorships for a defined geographic area or affiliates.
• Manages the community benefit activity for the geography with local community, school and government, not-for-profit agencies in area of responsibility.
• Works collaboratively with the Director to create strategic outreach plans for engaging elected officials and educating them about Community Benefits efforts across the OU.
• Works across various departments within Sutter Health, including Finance, Compliance, Legal, Facility Development, Affiliate Senior Management, Communications and Business Development, to ensure Sutter Health’s standing in the community is strong and that our work is being shared appropriately.
• Advances and manages community-wide strategies around identified strategic priorities and focus areas.


People Management
• Management and oversight of the Community Benefit program and assigned resources. This includes the evaluation of existing resources, gap analysis to skills/needs, and identification and recommendation for full staffing.
• Conducts annual performance evaluations
• Recruits, interviews and selects staff
• Manages the relationships with multiple community organizations and public officials on behalf of our Sutter Health affiliates.
• Works directly with community-based non-profit groups to conduct needs assessments and develop programs to meet community health needs.


Financial Administration, Data & Reporting
• Assists Director in continued funding through working with Philanthropic Foundation and researching Grant opportunities and writing proposals for funding.
• Manages the documentation and reporting of all affiliate community benefit activities and for providing quarterly reports to Sutter through utilizing the CBISA system.
• Assists in the creation of proposals for grants to Federal, state and County organizations. Develops programs and implementation plans. Works in partnership with Medical staff, Social Workers, Community Program leaders, etc. to implement programs and evaluate performance.
• Manages the required annual community benefit report as required by Office of Statewide Health Planning and Development, and coordinates with by system External Affairs office to standardize reporting across the system and OU.
• Manages the process of data and programmatic outcome collection and reporting for defined geographic areas or affiliates.
• Manages the Community Benefit budget for the defined geographic areas or affiliates and works with the Community Benefit Coordinator and Associate to ensure an annual payment schedule is created and adhered to.
• Reports to affiliate administrative teams and management teams on community benefit activities throughout the year.
• Present Community Benefit and Government Relations updates to affiliate Leadership teams, A-team and Management Team meetings when appropriate.


Leadership and Professional Accountability
• Manages projects that are developed and implemented through the Community Benefit Department
• Manages Community Benefit Department Grants and Sponsorships to community-based non-profits serving the poor & uninsured
• Develops the annual Community Benefit Report to the community regarding OU charity care and quantifiable benefits to the broader community
• Monitors federal, state and local regulatory reporting on community benefit activities. Manages local level reporting that is county specific, for example, all Development Agreements and local Charity Care Reports (i.e. CPMC).
• Serves as OU's representative to key community coalitions, meetings and other forums.
• Works across various Sutter Health departments, including but not limited to: Affiliate Senior Management, Facility Planning and Development, Care Coordination, Communications, Business Development, Finance, Compliance, Legal and on collaborative strategies with all operating unit medical foundations.
• Provide support and strategy for community relations activities and serve as a visible member of the Sutter Health team, building external relationships and acting as a liaison between Sutter Health and the greater community.
• Provides hands on coordination assistance to community benefit initiatives in the local communities.
• Provides a link for the OU to system wide Community Benefit activities.



EDUCATION:
Equivalent experience will be accepted in lieu of the required degree or diploma.

Bachelor's: Public Policy, Public Health or Public Administration, or a related field



TYPICAL EXPERIENCE:


8 years recent relevant experience


SKILLS AND KNOWLEDGE:

Must be ale to manage multiple projects and deadlines.

Must have a significant understanding of community-based organizations and local and statewide Foundations and government agencies.

Knowledge of public relations and event coordination.

Knowledge of Microsoft Word, Excel, PowerPoint, and Outlook software.

Excellent interpersonal and written and verbal communication.

Demonstrated ability to work independently with minimal direction.

Demonstrated ability to effectively work with executives and senior management teams.
