Your Role as Mental Health Clinician - Mental Health Multidisciplinary Team:

As a Mental Health Clinician, you will provide assessment, brief intervention, case management and coordinated care to adults with complex mental health needs. Working alongside GPs and a multidisciplinary team, you will deliver recovery-oriented, trauma-informed and culturally safe support that empowers people on their recovery journey.

Key responsibilities include:

  • Conducting comprehensive mental health assessments and develop individualised, recovery-oriented treatment plans in partnership with participants and their care team.

  • Delivering evidence-based psychological and psychosocial interventions that support recovery, wellbeing and improved outcomes.

  • Providing case management and care coordination for participants with complex mental health needs across their care journey.

  • Partnering closely with GPs and other healthcare providers to deliver shared care, clinical consultation and coordinated support.

  • Facilitating referrals and connect participants with appropriate health, community and support services.

  • Collaborating with a multidisciplinary team to provide integrated, person-centred, trauma-informed and culturally safe care.

  • Maintaining accurate clinical records, manage risk and safety planning, and ensure compliance with clinical governance and quality standards.

What We’re Looking For

  • Qualified in Social Work, Psychology, Occupational Therapy, Counselling, Mental Health Nursing or a related discipline.

  • Experience in supporting people with complex mental health needs through assessment, intervention and case management.

  • Skills in risk assessment, safety planning and evidence-based, recovery-focused practice.

  • Comfortable working both independently and as part of a multidisciplinary team.

  • Ability to build strong relationships with clients, families, GPs and community partners.

  • Strong communication, time management and clinical documentation skills.

  • Commitment to delivering person-centred, trauma-informed and culturally safe care.

A Valuable Addition, Yet Not a Must-Have!

  • Experience working in primary care, shared care or general practice settings.

  • Knowledge of PHN-funded, stepped care or community mental health programs.

  • Experience supporting people transitioning from acute or hospital-based mental health services.

  • Connections with and understanding of First Nations, CALD, Pasifika and LGBTQIA+ communities.

  • Lived or living experience of mental health recovery.

Essential Requirements

  • Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)

  • Relevant probity checks required by legislation and WMQ Limited policy.

  • Current Driver’s Licence and willingness to travel for work
