This full-time opportunity is ideal for a compassionate clinician who enjoys building meaningful relationships with older adults while delivering personalized rehabilitation services that promote independence, mobility, and long-term wellness.
This role offers an alternative to high-volume rehabilitation settings by emphasizing quality patient care over quantity. The successful candidate will work alongside an interdisciplinary healthcare team focused on restoring function, improving mobility, reducing fall risk, and helping seniors remain safely engaged within their communities.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
- Deliver evidence-based rehabilitation interventions that improve mobility, balance, strength, endurance, and functional independence.
- Monitor patient progress and adjust treatment plans based on clinical outcomes and patient goals.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and interdisciplinary care teams.
- Educate patients and caregivers on therapeutic exercises, injury prevention, home safety, and wellness strategies.
- Maintain timely, accurate, and compliant clinical documentation.
- Participate in interdisciplinary care conferences and contribute to coordinated treatment planning.
- Promote patient safety while supporting evidence-based rehabilitation practices.
- Build trusted relationships with patients and families through compassionate, individualized care.
- Support quality improvement initiatives that enhance patient outcomes and rehabilitation excellence.
Required Qualifications
- Current California Physical Therapist (PT) license or eligibility to obtain California licensure before employment.
- Doctor of Physical Therapy (DPT) or equivalent accredited Physical Therapy degree.
- Strong clinical assessment and treatment planning skills.
- Experience treating adult or geriatric patient populations.
- Excellent communication, patient education, and interpersonal abilities.
- Ability to work collaboratively within interdisciplinary healthcare teams.
- Strong organizational skills with exceptional attention to patient outcomes.
- Must currently reside within commuting distance of the Los Angeles Metro area or be prepared to relocate independently before employment.
- Authorized to work in the United States.

Preferred Qualifications
- Experience working with medically complex geriatric patients.
- Background in outpatient rehabilitation, community-based rehabilitation, home health, or senior-focused healthcare programs.
- Experience with balance training, neurological rehabilitation, fall prevention, and functional mobility programs.
- Knowledge of Medicare documentation and rehabilitation compliance.
- Board Certification or advanced rehabilitation training.
- Experience collaborating within interdisciplinary healthcare models.
- Stable employment history demonstrating progressive clinical growth and patient care excellence.
HireNow Package
- Compensation: $115,000–$136,000 annually
- Benefits: Comprehensive medical, dental, and vision insurance; up to seven (7) weeks of paid time off; paid holidays; sick leave; 401(k) with employer matching; continuing education opportunities; professional development; bonus eligibility where applicable; and additional company-sponsored benefits.
