PRN | Hospice | Wasatch Back | Mileage Reimbursement Provided
We are looking for a compassionate and dependable Medical Social Worker to join our growing hospice team on a PRN basis, serving patients and families throughout the Wasatch Back, including the Park City area.
Hospice social work is about meeting patients and families where they are; emotionally, socially, and practically. Our social workers are an important part of the hospice team, helping patients and loved ones navigate the challenges that can come with serious illness and end-of-life care.
We have a team that genuinely cares about the people we serve, and we are looking for someone who shares that commitment. We want a social worker who will listen, advocate, provide reassurance and resources, and help patients and families feel supported during an incredibly meaningful time in their lives.
This is a field-based position involving travel to patients in their homes and residential care settings throughout the Wasatch Back. Mileage is reimbursed for work-related travel.
What You’ll Do:
As a Hospice Medical Social Worker, you will work closely with patients, families, caregivers, and the interdisciplinary hospice team to identify needs and provide psychosocial support throughout the hospice journey.
- Complete initial and ongoing psychosocial assessments of patients and families.
- Identify emotional, social, financial, environmental, and other needs that may affect the patient or family.
- Provide supportive counseling to patients and families related to serious illness, end-of-life concerns, grief, loss, and changing family dynamics.
- Provide short-term crisis intervention and support when needed.
- Help patients and families identify available community resources and connect them with appropriate services.
- Advocate for the needs, wishes, dignity, and quality of life of patients and families.
- Assist patients and families with planning, problem-solving, and accessing available resources.
- Collaborate with nurses, physicians, chaplains, aides, and other members of the interdisciplinary hospice team.
- Participate in interdisciplinary group (IDG) meetings and contribute to the development and ongoing evaluation of the patient’s plan of care.
- Communicate changes in patient or family needs to appropriate members of the care team.
- Provide bereavement support and participate in bereavement services as appropriate.
- Maintain relationships with community agencies and resources to help meet patient and family needs.
- Complete accurate and timely documentation of assessments, visits, telephone contacts, interventions, and referrals.
- Attend team meetings and other clinical meetings as appropriate.
- Maintain patient confidentiality and follow hospice policies, professional standards, and applicable regulatory requirements.
What We’re Looking For:
- Degree in social work from an accredited program and qualifications appropriate for providing social work services in the hospice setting.
- MSW preferred.
- Healthcare social work experience preferred; hospice, palliative care, home health, hospital, geriatric, or end-of-life experience is especially valuable.
- Strong assessment, counseling, communication, and interpersonal skills.
- Compassionate and comfortable supporting patients and families experiencing serious illness, death, grief, and loss.
- Ability to work independently in the field while collaborating closely with the interdisciplinary team.
- Good judgment and the ability to respond appropriately to challenging or crisis situations.
- Strong organization, time-management, and documentation skills.
- Respect for patients and families from different cultural, spiritual, religious, and social backgrounds.
- Knowledge of community resources and the ability to help families navigate available services.
- Dependable, flexible, and responsive to the needs of patients, families, and the care team.
- Current driver’s license, automobile insurance, and reliable transportation.

We also value someone who communicates well with the rest of the hospice team. Social workers often recognize needs that may not be immediately visible to others, and your perspective is an important part of providing whole-person hospice care.
About the Position
This is a PRN position serving hospice patients and families throughout the Wasatch Back, including the Park City area.
Because this is a community-based position and travel between patient locations is required, mileage reimbursement is provided for work-related travel.
You will have the independence of working in the community while still being part of an interdisciplinary team of nurses, aides, chaplains, physicians, and other professionals who share the same goal: supporting patients and families with comfort, dignity, compassion, and respect.
